Description
 Statement of Work THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION AND A SEPARATE SOLICITATION WILL NOT BE ISSUED. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6 under Simplified Acquisition procedures in Subpart 13.5 as supplemental with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this acquisition is RFQ107410 and is being issued as a Request for Quote. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-25. This acquisition will be processed under Simplified Acquisition Procedures. The associated North American Industry Classification System (NAICS) Code is 621330. INTRODUCTION The District of Columbia Program for AIDS Progress is a collaborative effort between the National Institutes of Health and the District of Columbia Department of Health, with the collaboration of the George Washington University School of Public Health and the GW HIV/AIDS Institute. The goals of the project are to reduce the burden of HIV/AIDS in the District of Columbia, to produce research in the areas of HIV/AIDS prevention and treatment, and to build infrastructure that will have long lasting impact on improving health in the nation’s capital. SCOPE OF WORK The physician consultant will report to the Chief, Critical Care Medicine Department (CCMD), CC, NIH on behalf of the program leadership. The physician consultant will be designated to work at various project locations and will be assigned to support project leadership as deemed in the best interest of the program. SPECIFIC REQUIREMENTS The physician consultant will work with program leadership in the following areas according to leadership priorities: 1.Establishing communication with project stakeholders about goals, priorities, performance, and results of projects. Stakeholders would include but not be limited to DC government officials, NIH leadership, patient care providers, patient groups, community and civic groups. 2.Developing strategic plan and specific projects as directed by project leadership 3.Monitoring development and execution of specific projects as assigned by program leadership. 4.Assisting leadership in recruiting and evaluation staff for project. 5.Analyzing project data and preparing data for administrative presentations and scientific publications and presentations. 6.Assessing community programs in other geographical areas and recommending best practices or novel approaches to program leadership in order to attain program goals. QUALIFICATIONS The physician consultant must have the following qualifications 1.Board certified physician in a medical specialty relevant to HIV/AIDS 2.A minimum of 5 years of experience and progressive responsibility in an area relevant to HIV/AIDS. 3.Subject matter expertise in the broad area of HIV/AIDS 4.Experience managing HIV/AIDS projects 5.Demonstrated ability to produce and publish scientific research 6.Ability to communicate effectively with individuals and groups 7.Willingness to travel to all areas in the District during weekdays and weekends during normal business hours as well as evenings and some holidays and to travel to other regions nationally to acquire information for the project or to present information at appropriate forums. CONTRACTOR REQUIREMENTS FOR DELIVERY OF SERVICES This position will be a contract job at 50% effort. Salary and benefits will be commensurate with the qualifications of the successful candidate and local standards. The Contractor shall be required to document actual hours worked by use of a timesheet. The Contractor shall provide this documentation in a format (preferably electronic) which is an easily audited format acceptable to the Government. ABSENCES FROM WORK The Contractor shall notify the Chief, Critical Care Medicine Department, CC, NIH at least 5 business days in advance of any absence from performance of services. In the event of an emergency or unexpected need for absence, the Contractor shall notify the Chief, CCMD immediately. Excessive absences from providing services may result in the termination of this contract. SUPERVISORY CONTROLS The Contractor serves under the general direction of the Chief, CCMD. Duties and responsibilities are carried out with independence and do not require direct supervision. Contractor refers complex or potentially controversial matters to the Chief. Performance is reviewed for compliance with established program outcomes, and overall contributions to the Program in achieving desired goals. GUIDELINES Guidance includes District of Columbia Department of Health, NIH, Department of Federal Policies and References as well as standards of practice set forth by the JCAHO, OSHA and the National Institute for Occupational Safety and Health. Interpretation and adaptations of the guidelines to specific problems are frequently required. PHYSICAL DEMANDS Work is primarily sedentary with some walking, standing and bending, and lifting of objects such as files, reports, books, etc. May require to work some nights, evenings, weekends and holidays, and some shifts will be more than 8-12 consecutive hours. PERFORMANCE MEASUREMENTS Evaluations of the Contractor’s performance will be conducted periodically throughout the contract period of performance. Performance will be measured against those standards set forth in the statement of work and elsewhere in the contract as well as those set standards in the NIH Contractor Performance System. Poor or unsatisfactory performance may result in discontinuance of part or the entire contract. Formal performance reviews will be conducted at least annually in accordance with procedures set forth in the NIH Contractor Performance System. LOCATION OF CONTRACT ACTIVITIES The Contractor will be required to provide services throughout the Baltimore and Washington metropolitan areas. PERIOD OF PERFORMANCE AND SALARY The period of performance for this contract shall be 12 months. The Contractor will not receive any “holiday pay” should they be required to work on a federal holiday. The Contractor is allotted 6.5, eight hour days of paid annual leave and 6.5, eight hour days of paid sick leave if needed. EVALUATIONS The Government shall evaluate the technical and cost proposals and an award shall be made using Best Value award criteria. The Contractor’s technical proposal will be evaluated based upon the following technical criteria. These criteria are listed in order of relative importance. •Past Performance – This item refers to the Contractor’s performance of similar tasks in previous contract efforts. These contracts must be completed within the past three (3) years or currently in progress for services similar to the requirements of this statement of work. (35%) •Experience of proposed individuals – Resumes are required for key personnel proposed. Resumes should contain details (length of experience, depth of experience, dates employed and employer) which specifically address the skills and experience contained in the Statement of Work. This item refers to “Experience and Skill” listed under each subtask and in those past performance references cited. (35%) •Technical and management approach – This item refers to the manner in which the contractors propose to provide the support required. (10%) •Cost – This criteria applies to the overall cost for the requirement. (20%) TRAINING Training, travel, conference attendance is subject to approval by the Chief, Critical Care Medicine Department. Requests will be addressed on a case by case basis. IDENTIFICATION BADGES AND KEYS The Government will provide Contractor with an identification badge which shall be worn at all times while on duty. The identification badge will contain a photograph of the employee, with employee name and an expiration date. The Government will issue keys to Contractor as appropriate. The Government will issue a parking permit to Contractor to allow for parking on the NIH campus. Contractor shall be required to return identification badge, keys, and parking pass to the Government immediately upon termination of employment. The Contractor shall not invoice the Government for the employee's last pay period until identification badge, keys, and parking pass are returned. SECURITY Any personal data collected and/or used under this contract is subject to the Privacy Act Clause (FAR 52.224-2). The Contractor will use any Privacy Act information provided by the Government or other Government contractors for the purposes authorized by this contract only and for no other. The Contractor will not provide the information to any other person or entity, unless explicitly directed. Each person providing service under this proposal will be required to sign a document of understanding stating that he/she understands and will comply with Privacy Act requirements. 508 COMPLIANCE Section 508 requires that Federal Agencies’ electronic and information technology is accessible to people with disabilities. The Federal Acquisition Regulations (FAR) Final Rule for Section 508 (Accessibility) can be found at: http://www.section508.gov. Unless it is an “undue burden” or compliant products or services do not exist, the proposal must comply with Section 508. The Contractor should state that they would comply with the requirements of Section 508 or cite a justifiable reason for an exception. If any additional costs for compliance are anticipated, these costs should be identified. For existing systems, the Contractor may not be able to identify all costs for compliance if the Contractor did not develop that system, but should state that any development performed during the course of this task order will be compliant. Within 30 days of this task award, the Contractor should provide a plan for compliance for the system as a whole, including any costs associated with compliance. MALPRACTICE INSURANCE Malpractice insurance will not be covered under this contract. The Contractor is responsible for their own malpractice insurance. Parties responding to this solicitation may submit their offer in accordance with their standard commercial practices (e.g. on company letterhead, formal quote form, etc) but must include the following information: 1) company?s complete mailing and remittance addresses 2) discounts for prompt payment if applicable; 3) Dun & Bradstreet number; 4) Taxpayer ID number; 5) Catalog or Published Price Listing applicable to the service; 6) Offerors must meet specifications as noted in the synopsis. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications/Commercial Items with their offer. Note: Contractor must be registered with the Central Contractor Registration (www.ccr.gov). The Government intends to make a single award based on the lowest price technical accepted offeror to the eligible, responsible offeror whose offer conforms to the solicitation, and is the most advantageous to the Government. The Government reserves the right to make an award to that offeror whose quotation provides the best overall value to the Government. Interested offerors may submit proposals electronically only within 15 days from the posting of this notice no later than 7/5/2008. Eastern Standard Time 5:00 p.m. Companies that sent in proposal may do so again. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications/Commercial Items with their offer. Note: Contractor must be registered with the Central Contractor Registration (www.ccr.gov). The Government intends to make a single award based on best value to the Government, technical and price factors considered. The anticipated award date for this requirement shall be on or before 07/18/2008.
