- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and written solicitation will not be issued. This solicitation is being issued as a Request for Proposal. The provisions and clauses incorporated herein are those in effect through Federal Acquisition Circular 2005-25, effective May 22, 2008. This solicitation is unrestricted the NAICS code is 611430 and the small business size standard is $5.0 million. The Department of Veterans Affairs/Employee Education Resource Center, Cleveland, Ohio proposes to enter into a non-competitive commercial item contract with the Amputee Coalition of America Inc., 900 E Hill Avenue, Knoxville, Tennessee to provide: an existing, instructor led course, to be provided at identified designated VA sites. The Peer Visitor Train-the-Trainer program will certify VA staff as peer visitor trainers, who are then able to train volunteer people with amputations of functional limb loss (PWLL) to be peer visitors. One VA employee will be trained from each VISN by the end of July 2008. Training materials will be provided to trainers. The training will be a 3-day training seminar, for 7 train-the-trainer candidate and 15-20 PWLL participants. The procured Peer Visitor Train-the-Trainer program shall be readily available for immediate implementation. 1. Training Three (3) day training seminars; 2 instructors, 9 days; 2. Trainer manuals for seven (7) students per site (3 sites) 21 each; 3. Peer visitor manuals, 811 each;. 4. Training supplies for 3 sites; 5. Educational materials and on e(1) year Amputee Coalition of America membership for 21 Veterans Integrated Service Networks (VISN). Proposal must include shipping costs, instructors transportation, lodging and any other costs associated with the training seminar(s). The objective of the Train-the-Trainer National Peer Visitation Program is to provide each Regional Amputation Center (RAC) with trained and certified staff who can train volunteer Peer Visitors (PV) for each RAC to: 1) Enhance the current system of care moving from a disease model to a wellness model; 2) Provide the education needed for selected staff to become certified Peer Visitor Trainers; 3) Ensure compliance with the Amputation System of Care requirement that each Regional Amputation Center develop a Peer Visitation Program by the end of July 2008. Delivery of Peer Visitor Training in 3 Regional VISN host sites: one in the East Region, the West Region and the Central Region. The Amputee Coalition of America shall provide the course in 3 Regional VISNs in a train-the-trainer (T-T) format. Two day of training for 7 trainer candidates, followed by one day for trainer candidates to train 15-20 volunteer Peer Visitors under supervision. Host VISN site selects training dates, location, space, 7 trainers for each class (one from each VISN in the region) and 15-20 people with limb loss (PWLL) for each class. Amputee Coalition of America shall provide each trainer with trainer manuals. Amputee Coalition of America shall provide a point-of-contact for Host VISN coordinator for communication about project coordination. Amputee Coalition of America shall provide 2 instructors for each of the 3 courses, for 3 days of training for each region. Peer Visitor manuals Limb Loss Library Materials & ACA Membership for 21 VISNS The project implementation should begin no later than one week from date of contract award. Delivery Schedule: ProjectApproval ACA will contact the individual identified as the Host VISN Coordinator (Task 3.1) for each site1 week after award of contract Delivery of Course to 3 Select Host VISNs- (Task 3.1) one in each region (east, west and central) with 7 per class1st course begins 3 weeks after award of contract Amputee Coalition of America (ACA) will provide 2 instructors to train 7 trainers per class per regionAs established by each selected host VISN for the duration of the course offering Amputee Coalition of America will provide education manuals to each trainer3 weeks after award of contract and prior to each course. Amputee Coalition of America (ACA) will provide Peer Visitor (PV) manuals.3 weeks after award of contract and prior to each course. ACA will provide educational materials and 1 year ACA membership for 21 VISNs3 weeks after award of contract.
