This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W7795181500003 is issued as a Request for Quote (RFQ). This request for Quote is due by 20 JUNE 2008, 12:00pm (Pacific local time). The RFQ will result in a firm fixed price contract. This solicitation is 100% set aside for small business. Need OSHA compliant dental treatment for dental treatment of class 3s to class 2 for a minimum of 200 soldiers and a maximum of 400 soldiers. (Please note there will not be a set number of soldiers per day, although one could reasonably anticipate a minimum of 20 soldiers per day.) The exception to this will be the first two and a half days. We anticipate 53 soldiers starting at 1000 on 10 July, 60 soldiers on 11 July and 25 soldiers on 12 July that will require treatment. 2. Expect contractor to provide sufficient personnel and equipment to accomplish the requested number in the time specified. Please note: an oral surgeon will need to be on location to perform extractions, root canals and other operative procedures. 3. Dentists must accept the Form 603 (written report from the last dental exam); however, they are allowed a brief exam to confirm what they see in the mouth with what is on the Form 603. If the x-rays are in the dental record the soldier brings with him/her to the appointment, those x-rays are to be used. However, the treating dentist will be authorized to take additional periapical x-rays if the information on the 4 bitewings is not sufficient for planning treatment. Panographic x-rays must be approved in advance by the Contracting Officers Representative (COR). a. The following dental procedures will only be authorized for restorative services: D2140 Amalgam One Surface D2150 Amalgam Two Surfaces D2160 Amalgam Three Surfaces D2330 Composite One Surface D2331 Composite Two Surfaces D 2332 Composite Three Surfaces D2335 Composite Four or More Surfaces D2391 Composite One Surface D2392 Composite Two Surfaces D2393 Composite Three Surfaces D2394 Composite Four or More Surfaces D2799 Provisional Crown D2931 Prefabricated Stainless Steel Crown D2950 Core Buildup, including any pins D2954 Prefabricated Post and Core Posterior composites will be authorized only when a posterior composite is necessary to provide a quality restoration and no other alternative is available. Posterior composites must be approved in advance by the COR. b. The following dental procedures will only be authorized for endodontic services: D3310 Anterior Root Canal Therapy D3320 Bicuspid Root Canal Therapy D3330 Molar Root Canal Therapy D3346 Anterior Root Canal Retreatment D3347 Bicuspid Root Canal Retreatment D3348 Molar Root Canal Retreatment c. The following dental procedures are authorized for periodontal services: D4355 Full Mouth Debridement D4342Periodontal Scaling and Root Planing, 1 to 3 teeth per quadrant Periodontal scaling and root planning is authorized only when a periodontal abscess is present and must be approved in advance by the COR. d. The following dental procedures are authorized for prosthodontic services: D5110 Complete Denture-Maxillary D5120 Complete Denture-Mandibular D5211 Maxillary Partial Denture-Resin Base D5212 Mandibular Partial Denture-Resin Base e. The following dental procedures are authorized for oral surgery: (local anesthetic is the only anesthetic allowed) D7140 Extraction, Erupted Tooth or Exposed Root D7210 Surgical Removal of Erupted Tooth D7220 Removal of Impacted Tooth, Soft Tissue D7230 Removal of Impacted Tooth, Partial Bony D7240 Removal of Impacted Tooth, Completely Bony f. The following dental procedures are authorized for orthodontic services: D8680 Orthodontic Retention g. The following dental procedures are not authorized: Bridges and/or implants Removal of Asymptomatic third molars Routine Prophylaxes (cleanings) Restorations for aesthetics only Restorations for small (non-class 3) caries Extensive Root plane/scaling Osseous Surgery The treatment provided under this contract is to get the Soldier from a dental class 3 to a dental class 2 only. Comprehensive treatment (to dental class 1) is not authorized. h. X-ray if needed 10. The Contracting Officers Representative will review any dental treatment plan for dental care which exceeds $1000.00 per each soldier to ensure that the above guidelines are met. Exceptions can be made, but they must be approved in advance by the COR. If approval has not be given in writing, no more than $1,000 will be paid for any soldier receiving more than $1,000 worth of dental work. Definitions of dental classifications for oral health: a. Class 1: Patients with current dental examinations, which do not require dental treatment or reevaluation. Class 1 patients are worldwide deployable. b. Class 2: Patients with current dental examination, which require non-urgent dental treatment or reevaluation for oral conditions, which are unlikely to result in dental emergencies within 12 months. Class 2 patients are worldwide deployable. Patients in dental class 2 may exhibit the following: (1) Treatment or follow-up indicated for dental caries or minor defective restorations that can be maintained by the patient. (2) Interim restorations or prosthesis that can be maintained for a 12 month period. This includes teeth that have been restored with permanent restorative materials for which protective cuspal coverage is indicated. (3) Edentulous areas requiring prosthesis but not on an immediate basis. (4) Periodontium that (a) Requires oral prophylaxis (b) Requires maintenance therapy (c) Requires treatment for slight to moderate periodontitis and stable cases of more advanced periodontitis. (5) Unerupted, partially erupted or malposed teeth that are without historical, clinical or radiographic signs or symptoms of pathosis, but which are recommended for prophylactic removal. (6) Active orthodontic treatment. The provider should consider placing the patient in passive appliances for deployment up to six months. For longer periods of deployment, the provider should consider removing active appliances and placing the patient in passive retention. (7) Temporomandibular disorder patients in remission. The provider anticipates the patient can perform duties while deployed without ongoing care and any medications or appliances required for maintenance will not interfere with duties. c. Class 3: Patients who require urgent or emergent dental treatment. Class 3 patients normally are not considered to be worldwide deployable. (1) Treatment or follow-up indicated for dental caries, symptomatic tooth fracture or defective restorations that cannot be maintained by the patient. (2) Interim restorations or prostheses that cannot be maintained for a 12 month period. (3) Patients requiring treatment for the following periodontal conditions that may result in dental emergencies within the next 12 months. (a) Edentulous areas or teeth requiring immediate prosthodontic treatment for adequate mastication or communication or acceptable esthetics. (b) Active progressive moderate or advanced periodontitis (c) Periodontal abscesses (d) Progressive mucogingival condition (e) Periodontal manifestations of systemic disease or hormonal disturbances (f) Heavy sub gingival calculus (4) Edentulous areas or teeth requiring immediate prosthodontic treatment for adequate mastication or communication or acceptable esthetics. (5) Unerupted, partially erupted or malposed teeth with historical, clinical or radiographic signs or symptoms of pathosis that are recommended for removal. (6) Chronic oral infections or other pathologic lesions including: (a) Pulpal, periapical or resorptive pathology requiring treatment. (b) Lesions requiring biopsy or awaiting biopsy report. (7) Emergency situations requiring therapy to relieve pain, treat trauma, treat acute oral infections or provide timely follow-up care (e.g., drain or suture removal until resolved.) (8) Acute temporomandibular disorders requiring active treatment that may interfere with duties. c. Class 4: Patients who require periodontic dental examinations or patients with unknown dental classifications. Class 4 patients normally are not considered to be worldwide deployable. CLIN: 0002 DENTAL EXAMS COST: EXAM WITH PANO___________ COST: EXAM WITHOUT PANO___________ COST: PANO ONLY_________ SERVICES REQUIRED FOR DENTAL EXAMS: 1. Need OSHA compliant dental exam to determine dental classification for approximately for a minimum of 350 and maximum of 900 soldiers. (Please note there will not be a set number of soldiers per day, although one could reasonably anticipate a minimum of 20 soldiers per day.) This dental exam will conform to AR 40-501 standards, including bitewing and pano x-rays as required. In addition to the above numbers, there may be some soldiers who only need a panograph x-ray to make their dental records ready for mobilization. The exception is the first two and a half days where we expect 400 soldiers during that time period. We expect 175 starting at 1000 on 10 July, 200 starting at 0700 on 11 Jul and any remaining soldiers at 0700 on 12 July. 2. Expect contractor to provide sufficient personnel and equipment to accomplish the requested number in the time specified. 3. Documentation of all exams must be entered into DENCLASS same day of event. Soldiers must leave dental exam with a copy of all required dental documents in their dental record. CLIN 0003: HOURLY RATE WHEN NOT PERFORMING SERVICES UNDER CLINS 0001 AND 0002. $__________ THIS CLIN WILL ONLY BE UTILIZED IF THE CONTRACTOR WILL NOT BE PERFORMING EITHER DENTAL TREATMENTS OR EXAMS DURING THE 0700-2400 PERIOD. 1. PERIOD OF PERFORMANCE: Anticipated event times are 0700-2400 on a daily basis. The contractor will work 7 days a week from 10 JULY THROUGH 02 AUGUST 2008. This does not include set up and tear down time. Expect to be able to run continuously throughout this time period. Staff will need to stagger meal times to allow continuous operations. (Please note: should work be completed by 31 July, there would be no requirement to remain on location the last two days. Leaving before 2 Aug would need to be coordinated with LTC Munsey or the 81st Bde Commander, COL Kapral.) 2 LOCATION: B-181 Armory, Yakima Training Center, 1221 Firing Center Rd, Yakima, WA 98901. 3. Documentation for all work done at this event must be placed in the soldiers dental records. If vendor needs copies of services rendered, they must be prepared to make those copies on their own. 1. CONTRACTOR MUST HAVE PROVIDE MOBILE VAN(S) capability for this event. Contractor should NOT assume there will be adequate power connection with local buildings and must bring their own generator. Must have panograph x-ray capability. 2. Any prescription drugs that cannot be provided by the contractor will be purchased at a local pharmacy on the WAARNG MEDCOM government purchase card. (This will be coordinated ahead of the event.) 3. A list of all soldiers seen by name, ssn, and dental classification after treatment will be provided to LTC Munsey either in person, by fax, or email. DENCLASS or MEDPROS input same day of service. COL Bowles the state dental officer will be on location to assist with this process. 4. All dentists must be licensed in the United States. 5. The contractor must invoice by name of soldier and list of treatment(s) for each day of performed services which will be reviewed by LTC Munsey. This will also include the number of hours per day under CLIN 0003. The contractor will be paid only for work performed. 6. POC is LTC Munsey at 253-405-5757, alternate POCs are SSG Scott Borkin at 253-318-6129 or SFC Rey Javier at 253-405-5536. Please coordinate with LTC Munsey if equipment is going to be dropped off on location prior to the event. AWARD WILL BE BASED ON OVERALL BEST VALUE TO THE US GOVERNMENT. (WHEN ANNOTATING THE UNIT PRICES, THE CONTRACTOR SHOULD ALSO INCLUDE THE CONTRACTORS PER DIEM RATES, COST OF RUNNING THE MOBILE VANS, ETC&) The offeror must be registered on the Central Contractors Registration database (CCR).
