- Amendment #4 to Solicitation VA-260-08-RP-0169 1) INSTRUCTIONS, CONDITIONS AND OTHER STATEMENTS TO BIDDERS/OFFERORS, section (a.5) is changed read as follows: Offerors shall submit their proposals and scanned copies of their Bid Guarantee Bonds electronically by 12:00 PM, 29 May, 2008 to Erik.Berg@va.gov. Original Bid Guarantee Bonds shall be sent to the address listed in Block 7. 2) STRUCTURAL DRAWINGS CLARIFICATIONS: 2.1 Sheet E2.1 - Electrical Demo Plan Add second floor south Stairwell #201 and Office #202 to scope of work. Demo all light fixtures, PA speakers, receptacles, and fire alarm devices located on ceiling and walls included in demo work that are indicated on sheet A2.1 per the demo notes on sheet E2.1. Demo Device Quantities: Stairwell #201 Light Fixture = 1 Light Fixture (Emergency) = 1 PA Horn Speaker = 1 (Stockpile for reuse) Smoke Detector = 1 Office #202 Light Fixture = 1 Receptacles = All Smoke Detector = 1 Magnetic Door Holder = 1 (This wall is not being demolished. Patch and refinish per specifications.) 2.2 Sheet E3.2 - Electrical Floor Plan Office #202 Add new receptacles (one per wall) and locate in new furred stud wall. Reconnect to existing circuit. Add one phone/data drop per detail sheet E3.1. Verify cable quantities at phone/data drop with owner. 2.3 Sheet E4.2 - Electrical Lighting Plan Stairwell #201 Add Qty four (4) Type "A" light fixtures as shown on sheet A12.1. Connect three (3) to existing lighting circuit and one to existing egress lighting circuit. Office #202 Add Qty two (2) Type "A" light fixtures as shown on sheet A12.1. Connect to existing lighting circuit and add occupancy sensor per detail on sheet E4.2. 2.4 Sheet E5.1 - Electrical Special Systems Plan Stairwell #201 Wall mount and reconnect existing PA horn speaker removed in demo phase. Locate above stairwell landing door. Add empty 3/4"conduit with pull cord in North stairwell wall stubbed to j-box in accessible space above t-bar to connect upper and lower floor fire alarm devices for future fire alarm work. 2.5 Sheet M2.1 - Mechanical Demolition Plan Add second floor Stairwell #201 and Office #202 to Fire Sprinkler system scope of work. 2.6 Sheet P3.1 - Plumbing Floor Plan Add second floor Stairwell #201 and Office #202 to Fire Sprinkler system scope of work. Extend existing sprinkler heads down to new T-bar ceiling. Test upper floor Fire Sprinkler system per specifications and NFPA-13. 3) Solicitation Questions: 3.1 Question: There is an access opening on the bottom of the south face of the building. It would be directly behind the new porch once completed. It that opening to remain as is, or is it to be removed and the opening sealed? Answer: The existing crawl space opening is to be replaced with a new interior access through CRAWL ACCESS 163 under the new stairs. The old access should be blocked. 3.2 Question: There are three trees and one stump adjacent to the south end of the building. Are these to be removed, and are the remaining stumps to be removed or ground down? If the stumps are to be ground down, to what depth? Answer: Remove the three existing trees/shrubs and grind stumps and remove roots to a depth required for installation of new ramp. 3.3 Question: Is the existing light pole at the sidewalk adjacent to the southwest corner of the building in the path of the new ramp to be installed? If so, who is to relocate the light pole and associated power line(s)? Answer: Per our calculations the existing light pole is not in the path of the new ramp and sidewalk, see sheet A3.1. The light pole base is the circle to the left of the ramp. 3.4 Question: Is the width of the existing corridor at the north end of the building wide enough to allow for a sealed access opening to the work site and still provide enough remaining space in the corridor to provide accessible access for travel along the corridor? Answer: The Corridor width is approximately 7 feet 8 inches. 44 inches must be maintained on the public side for traffic. 3.5 Question: Dwg ASB/L2, notes 1 & 2 suggest that lead testing has been performed while dwg. A2.1 Note B states the opposite. Has a lead survey been conducted in the areas of construction for this solicitation? Answer: Asbestos and Lead testing has been completed, the note on A2.1 is for undiscovered asbestos or lead material. 3.6 Question: On Dwg. ASB/L1 are the notes 1-7, 18, 21-25 all considered to be Asbestos Containing Materials? If so, are these notes limited to the specific areas that they are shown on the drawings? For example, is #21-23 limited to RM 152A -Waiting or considered to be throughout all areas? Answer: No, only items shown on ASB/L1 Legend indicated to be asbestos containing are flooring, wall mud/ texture and steam pipes. 3.7 Question: Has an Asbestos Survey been performed? If so, we all rooms in the construction area tested? Answer: Yes. All the different materials have been tested. See Specification Section 01569 fir more information, 3.8 Question: Is the overall height of the crawl space known? If so, what is that height? Answer: 30 to 32" and can vary. 3.9 Question: For Item # 3 under Technical Proposal, Accreditation/ Certification, can this capability be provided by the subcontractor and not the prime? Answer: If the Accreditation/Certification is required for an individual, then the subcontractor can provide. If the Accreditation/Certification is required for the business, then the prime contractor must provide. 3.10 Question: First, on the contract drawings, there is reference to a Post Indicator Valve on sheet P3.1 that is approximately 40 feet south and 18 feet east of B209S. There is no PIV at that location. The PIV for B209N/S and B210N/S is located north of B209N near the Canteen loading dock. Also, the fire line comes up in B209N. Answer: Correct 3.11 Question: The Contract Specifications call for inspection of the existing line for flow and compliance and if not, they are to be brought up to code. How can this be achieved if the flow test for B209S Phase 1 is not available, B209 and B210 are part of the same supply line, will there be upgrades to this building and what would they be, and who will give the authority to dig up the lawn on the backside of B209N to determine if the pipe is compliant prior to bid time? Answer: Flow test was performed for building 209 of the VA DOM. On December 6th, 2007. Location of test: Residual was taken from hydrant closet closest to the existing PIV supplying building 209. (Hydrant #12) static was taken from the next hydrant in line upstream of the residual hydrant (Hydrant #11). Elevation relevant to finished floor of 209 was set at 0'- 0" Results: Static: 74 PSI Residual: 54 PSI Flow: 1040 GPM Contact Contracting Officer for pre bid inspections. Contractor is to ensure area of construction is code compliant. 3.12 Question: Where would the vault for the backflow device be located? Answer: Contractor to determine and provide submittal for approval as part of the Fire System design required by specifications. 3.13 Question: What kind of vault, above grade or below grade? Answer: Provide vault submittal. Below grade with sump pump.
