Description: The National Institute of Standards and Technology (NIST) in Boulder, CO requires pest and rodent control services. The specific tasks will be described in the Performance Work Statement (PWS) and attachment of area maps. DESCRIPTION: THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL SERVICES PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6-STREAMLINED EVALUATION OF OFFERS AND SOLICITATION FOR COMMERCIAL ITEMS AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION AND IS A REQUEST FOR QUOTATION (RFQ); A WRITTEN SOLICITATION WILL NOT BE ISSUED. TELEPHONE REQUESTS WILL NOT BE HONORED AND A BIDDERS LIST WILL NOT BE MAINTAINED. ***This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-25. *** *** The associated North American Industrial Classification System (NAICS) code for this acquisition is 561710 with a small business size standard of $6.5. This requirement is a 100% small business set-aside, therefore, only small businesses may submit a quote. *** *** A firm-fixed-price purchase order is anticipated in accordance with the attached Performance Work Statement (PWS) as a result of this solicitation. ***The work shall be performed in accordance with the US Department of Labor Wage Determination No. 2005-2103 dated July 5, 2007. ***All interested vendors shall provide a quote for the following line items: LINE ITEM 0001: Description: Contractor shall provide all supervision, labor, material and equipment for pest and rodent control in accordance with the PWS and attached maps. Quantity: 12; Unit of Issue: months. Period of Performance: July 1, 2008 through June 30, 2009. LINE ITEM 0002: Description: Option Period 1 - Contractor shall provide all supervision, labor, material and equipment for pest and rodent control in accordance with the PWS and attached maps. Quantity: 12; Unit of Issue: months. Period of Performance: July 1, 2009 through June 30, 2010. LINE ITEM 0003: Description: Option Period 2 - Contractor shall provide all supervision, labor, material and equipment for pest and rodent control in accordance with the PWS and attached maps. Quantity: 12; Unit of Issue: months. Period of Performance: July 1, 2010 through June 30, 2011. LINE ITEM 0004: Description: Option Period 3 - Contractor shall provide all supervision, labor, material and equipment for pest and rodent control in accordance with the PWS and attached maps. Quantity: 12; Unit of Issue: months. Period of Performance: July 1, 2011 through June 30, 2012. LINE ITEM 0005: Description: Option Period 4 - Contractor shall provide all supervision, labor, material and equipment for pest and rodent control in accordance with the PWS and attached maps. Quantity: 12; Unit of Issue: months. Period of Performance: July 1, 2012 through June 30, 2013. *** Award shall be made to the vendor that provides the lowest price, technically acceptable (LPTA) quote. The Government will evaluate technical acceptability based on the following evaluation criteria: Technical capability: information in the quote that clearly documents that the offered services meets or exceeds the requirement in the PWS; Past performance to determine overall quality of the service provided: Evaluation of past performance will be based on the references provided IAW FAR 52.212-1(b)(10) and/or the offeror's recent and relevant acquisition history with NIST or its affiliates. In order to conduct a past performance review, offerors shall provide a list of at least three (3) contacts and the vendor, including phone number, contract number and amount of the contract; the address/telephone number of the Contracting Officer; and the date the services were completed; and Past experience: experience in providing pest and rodent control services per the PWS. ***The following FAR Provisions and Clauses apply to this acquisition and can be accessed at http://acquisition/far/index.html Provisions: 52.212-1, Instructions to Offerors - Commercial Items; 52.212-2, Evaluation-Commerical Items 52.212-3, Offeror Representations and Certifications - Commercial Items.
