- PERFORMANCE BASED WORK STATEMENT TOT TOWN DAY CARE FORT BENNING, GEORGIA 1 GENERAL 11 SCOPE OF WORK This is a non-personal service contract. The contractor shall provide qualified personnel to provide child care to Fort Benning Educational and Developmental Intervention Services (EDIS) for children residing on the military installation who are eligible for services under Part C of Public Law 102-119, Individuals with Disabilities Education Act (IDEA), October 7, Revised 2004 The children must be eligible beneficiaries of the Military Healthcare System as determined by the Medical Department Activity (MEDDAC) Commander. 111 HOURS OF PERFORMANCE. Childcare shall be provided Monday through Friday, 6:30 am. to 6:30pm 1.1.2.. The government reserves the right to verify the hours worked by the contractor by implementing sign-insignout procedures or any other means deemed necessary by the Contracting Officers Representative (COR) 11 3 Childcare services will not be required on Legal Federal Holidays. 1.2. PERSONAL APPEARANCE. Contractor personnel working under this contract shall practice high standards of personal hygiene and maintain a clean, neat appearance while on duty Clothing shall be clean and neatly pressed; free from lint, debris, dirt, mud, stains, or other substances; and void of rips, tears, and holes. 2 DEFINITIONS 2 1 CONTRACTING OFFICER: A person duly appointed with the authority to enter into, administer, and/or terminate contracts and make related determinations and findings on behalf of the government 2 2. CONTRACTING OFFICERS REPRESENTATIVE: A government employee selected and designated in writing by the contracting officer to act as his/her designated representative in administering a contract 2.3 QUALITY CONTROL: Those actions taken by the contractor to control the services provided to ensure they meet the requirements of the PBWS 2.4.. QUALITY ASSESSMENT AND IMPROVEMENT: Those actions taken by the government to check services to determine that they meet the requirements of the PBWS, requirements of the The Joint Commission (T JC), U.S Army Medical Command, and individual hospital medical staff quality assessment and improvement programs. 2.5. GOVERNMENT FURNISHED FACILITIES, SERVICES, SUPPLIES, AND EQUIPMENT 3 1. REGUI.ATIONS/D]RECTIVES/FORMS: UnTess noted otherwise, all required Army and Department of Defense Regulations, Directives and Forms as listed in Section C..6 can be found on the web 3 2. TRAINING: Educational and Developmental Tntervention Services (EDIS) personnel will provide training to Child care personnel to ensure activities and special needs of EDIS children are continued during placement of child in day care, A copy of this training will be placed in the childs day care file for future use 4. CONTRACTOR FURNISHED EQUIPMENT/MATERIALS. All personnel, equipment, supplies and materials necessary to finnish a full time quality day care. 5. SPECIFIC TASKS. The contractor shall provide child care to Fort Benning Educational and Developmental Intervention Services (EDIS) for children residing on the military installation, as requestedlordered by the COR or the Educational and Developmental Intervention Services Program Manager 5.1.. The contractor shall be in compliance with The Crime Control Act of 1990 and Department of Army Circular 690-92-1, 15 November 1992, Criminal History Background Checks, on individuals in child care services 5 2. The contractor shall provide a monthly invoice to the COR for all hours and partial hours care was rendered, within five (5) calendar days of the end of the month of service The invoice shall include a sign-in roster of all eligible children that used the service for the month and the number of hours they were at Tot Town each day during the month 5.3. Contractor shall ensure ratio of caregiver to children does not fall below 1 caregiver to 4 children birth to 15 Months and 1 caregiver to 5 children for children 15 months to 2 years 5.4. BENThllNG BUDDIES PROGRAM 5 4.. 1 The Bemting Buddies Program is an optional service provided by lot Town Day Care, which allows children to go to a day care setting which is more structured and provides class room instruction. 5 4.2. The Benning Buddies Program is typically used for children who can attend to and complete higher functioning skills. 5 4 3. The Benning Buddies Program offers 2 or 3 day classes and, dependent on the needs of the child and family, either class maybe recommended by EDIS staff and approved by the EDIS Program Manager.. 6. APPLICABLE REGULATIONS AND MANTJAL.S 6 1.
