- (i) This is a combined synopsis/solicitation prepared in accordance with the format in FAR Subpart 12.6 as supplemented by additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The information we request below is the minimum information required for us to make an award. If the applicable information is not provided entirely, your quotation will not be considered for award. (ii) This solicitation is being issued as a Request for Quote (RFQ) No. FA5205-08-Q-B001. Please submit a written quotation NLT 14 Apr 2008, 07:00 (Japan Standard Time) by Email. POC: 2Lt Charles Berry, Email: Charles.Berry@misawa.af.mil, Tel: 011-81-176-57-2443. (iii) This solicitation document and provisions/clauses are those in effect through Federal Acquisition Circular (FAC) 2001-27 (as of 28 Dec 04). (iv) This procurement is a Total Small Business Set-aside acquisition. The NAICS code 238990 applies to this solicitation. (v) Quotation is requested for the following services: Contractor will need to move a Play Systems by Child Works Model 6177 play structure in building 973 on Misawa Air Base. Play structure shall be taken apart, moved approximately 315 ft and reassembled to manufacture specifications and ASTM guidelines. Information on the structure can be found http://playsystems.superiorinternational.com/ Family Entertainment, Structure 6177. Moving path is flat smooth concrete with an average width of moving area of 15 ft, with one 4 ft wall with 8 ft entrance. All offerors must be able to read and understand manufacturer specification plans written in English. All offerors shall have all tools required to disassemble, relocate and reassemble play structure. Award will be considered on All-or-Non basis. Please provide the total price and a price breakdown, to include a list of items, labor hours, per diem, installation breakdown charges and travel cost (if any). (vi) The F.O.B. Point for this requirement is F.O.B. Destination to physical address (Misawa Air Base, Japan). Please provide warranty period. If your warranty is other than a standard commercial warranty, please provide a copy of your warranty information/certificate when responding to this solicitation. Please provide proposed delivery date and prompt payment discount terms if any. (vii) The provision at FAR 52.212-1, Instructions to Offerors' Commercial Item, applies to this acquisition. (viii) FAR 52.212-2 Evaluation- Commercial Items, Quotes/offers will be evaluated IAW FAR 13.106-2. Award will be made to the responsible offeror whose quote/offer represents the best value in terms of technicality (capability of item(s) offered to meet the government need) and price. (ix) FAR 52.212-4, Contract Terms and Conditions- Commercial Items applies to this solicitation. Addenda to FAR 52.212-4. FAR 52.212-4, para. (c), entitled 'Changes'. The following sentence is added to the end of para.(c): unilateral modification may be issued for administrative changes.
