Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Request for Quote is W9124B-08-T-0120 issued as request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acqui sition Circular 2005-16 dated 22 March 07. The associated NAICS code is 721110. 0001 MARRIAGE RETREAT DATES 20-22 MARCH 2008 24-26 APRIL 2008 22-24 MAY 2008 12-14 JUNE 2008 10-12 JULY 2008 0001AA Lodging fo r Marriage Retreat QUANTITY UNIT PRICE AMT 280 EA Double occupancy for 56 people per retreat. 56 persons which include 46 adults (20 couples and 6 facilitators) and approximately 10 children over the age of 3 yrs old. (Sliding sc ale to be designated for children over the age 3 yrs old who occupy a room with their parents). Approximately 26 rooms. Invoice actual usages. FORMULA: 56 x 5 Retreats = 280 EA. 0001AB Meals for Marriage Retreat QUANTITY UN IT PRICE AMT 280 EA Seven (7) Meals per person per retreat with complimentary beverages to include choices of water, tea, and coffee. 56 persons per retreat which includes 46 adults and approximately 10 children over the age of 3 yrs old. Child appropriate menu should be provided for children. (Child menu price to be designated for children over the age 3 yrs old). Final menus must to be agreed prior to final contract. Invoice actual usages. FORMULA: 56 x 1 Day x 5 Retreats = 280 EA Seven Meals Suggested schedule: Day One - hot lunch and hot dinner. Day Two - hot breakfast, boxed lunch and hot BBQ dinner. Day Three - hot breakfast and hot lunch. 0001AC Conference Rooms Per Marriage Retreat QUANTITY UNIT PRICE AMT 30 EA One conference room set -up into a classroom style to accommodate 46 adults with audiovisual equipment (podium, sound system, screen, TV, and DVD and VHS Players). Conference room shall have beverage service which includes coffee, tea, hot and cold water service for entire day. A nearby smaller or equivalent breakout room with at least 25 chairs. Both rooms must be available the entire three day retreat period. Invoice actual usages. FORMULA: 1+1=2 Rooms x 3 Days x 5 Retreats = 30 EA Suggested Beverage set-up Tim e: Day One - 1 pm - 6 pm Day Two - 9 am  12 pm Day Three  9 am  12 pm 0001AD Group Activities for Marriage Retreat QUANTITY UNIT PRICE AMT 35 HR Day Two: Challenge course activities to include professionally supervised high ropes activities or equivalent as per safety regulations and couple bu ilding activities for a block of 5 hours for 40 adults. The focus of the activities should be primarily couple building/communication orientated activities with some group teambuilding activities. Complimentary cold water to be provided throughout the ac tivities at approximately 1 pm to 6 pm per each retreat. An approximately two-hour evening campfire or an equivalent outdoor family oriented activity with hot/cold beverages, time to be determine according to weather conditions; inclusive of wo od or other required equipment and all necessary safety supervision requirements for the fire or equivalent activities. The schedule, programs and specific a ctivities to be agreed prior to final contract. If the activities are at an alternate site other than the conference location, contractor must provide transportation to and from the alternate activity site. Invoice actual usages. FORMULA: 5+2=7 HRS X 5 Retreats = 35 Hrs 0001AE Childcare Room/ Providers per Retreat QUANTITY UNIT PRICE AMT 1500 HRS Provide childcare room and two (2) certified child care providers to conduct approximately 20 hours of childcare for approximate ly 15 children. Below is the scheduled time for childcare. Provide water to children during care time. Proof of childcare certification must be submitted to the NTC Acquisition Command the next working day after contract award. Providers must be CPR ce rtified or have an equivalent certification. Providers need to provide child care, set-up, activities and equipment. Invoice actual usages. FORMULA: 15 children x 20 hours a retreat x 5 Retreats = 1500 Hrs. Childcare Proposed Schedule: Day One - 11-12 noon Day One - 12:15 pm - 6:15 pm Day Two - 8:45 am - 6:15 pm <B R>Day Three - 8:45 am - 11:45 am 0002 FAMILY DAY RETREAT 10 May 2008 07 June 2008 28 June 2008 26 July 2008 09 August 2008 0002AA Meals for Family Day Retreat QUANTITY UNIT PRICE AMT 300 EA Family Day Retreat for 60 people. Lunch and Evening meals with complimentary beverages per person to include coffee, tea, and water. Evening BBQ is recommended. Child appropriate menu should be provided for children. Child menu price to be designated for children over the age 3 yrs old. Final menus will be agreed upon prior to final contract. Invoice actual usages. FORMULA: 60 x 1 Day x 5 Retreats = 300 EA. 0002AB Family Day Retreat Activities QUANTITY UNIT PRICE AMT 300 EA Activities for 60 people with sliding scale for child participants. Children in childcare will not be charged for family day activities. The complete program should begin approximately 9 am and be completed by 6 pm. The focus of the activitie s should be primarily family relationship/communication building oriented activities with some group teambuilding activities. The full day activities will include but are not excluded to professionally supervise high ropes activities as per safety regula tions, nature walk, and other family building activities, campfire or equivalent evening outdoor activity with hot/cold beverages to include wood or other necessary equipment, all necessary safety supervision requirements should be met. Complimentary co ld water to be provided throughout the activities. The schedule, programs and specific activities to be agreed prior to final contract. Invoice actual usages. FORMULA: 60 person x 1 Day x 5 Retreats = 300 EA. 0002AC Childcare Room/Provid ers per Family Day QUANTITY UNIT PRICE AMT 800 HRS Provide childcare room and two (2) certified child care providers to conduct childcare for approximately 20 children approximately 9 am-6 pm exclusive of meal time but inclusive of set- up equipment. Providers need to provide child care activities and equipment. Providers must be CPR certified or have an equivalent certification. Proof of childcare certification must be submitted to the NTC Acquisition Command the next working day afte r award. Invoice actual usages. FORMULA: 20 children x 8 Hrs a Retreat x 5 Retreats = 800 HRS.
