Description

1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. 2. Solicitation number A08-39 is issued as a request for quotation (RFQ). All firms or individuals responding must be registered with the Central Contractor Registration (CCR), which can be found at http://www.ccr.gov/ . 3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-24. 4. This is a 100% Small Business Set-Aside. If there are no offers received from a small business concern, this acquisition for small business set-aside may be dissolved and the acquisition open to all responsive bidders. NOTE: This acquisition will not be reposted to the Electronic Posting System if the acquisition is dissolved for 100% small business consideration. The NAIC code is 423450 and the size standard is 500 Employees. 5. This solicitation contains six (6) line items as follows: MAC 5500 - P/N MAC55-CAAAXX-AXXEXX (Including MAC5500 Color, Standard Paper, North American, Acquisition Module AHA, Mobile Link UHS (Air Fortress)), Qty: 1. MAC 3500/MAC 5500 System Trolley - P/N 2017210-001, Qty: 1. Memory Card Reader Kit, Eng. - P/N 2027792-001, Qty: 1. MUSE Secure Digital Upgrade Kit, Eng. - P/N 2027777-001, Qty: 1. MobileLink Wireless Communication Installation Manual - P/N 2020299-021, Qty: 1. MAC 5500 Applications Training Kit - P/N 2028776-001, Qty: 1. All items must be manufactured by General Electric (GE). 6. FOB Destination for delivery to VA Medical Center-Manchester, 718 Smyth Rd. Manchester, NH 03104. To be delivered no later than 30 days ARO. 7. The provision at FAR 52.212-1, Instructions to Offerors, applies to this acquisition. Multiple offerors and awards are hereby deleted from this solicitation. All quotes must be for all items, as stated, partial quotes will not be considered. Award will be made to a single vendor. 8. The provision at FAR 52.212-2, Evaluation of Commercial Items applies. Quotes will be evaluated on price related factors only. 9. Offerors must submit a completed copy of the provision at FAR 52.212-3, Offeror Representation and Certifications Commercial, with its offer. FAR 52.212-3 can be downloaded from the internet: http://farsite.hill.af.mil/. Offerors that fail to furnish the required representation information, or reject the terms and conditions of the solicitation, may be excluded from consideration. 10.
