Description
DESCRIPTION: This is a combined synopsis/solicitation for a commercial service prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation Number VA-248-08-RQ-0188 is being issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-22. This requirement is 100% set-aside for Service Disabled, Veteran Owned Businesses (SDVOB) under NAICS Code 562112, Size Standard of $11.5 million dollars. PERIOD OF PERFORMANCE: Contract performance will cover the period April 1, 2008 through September 30, 2008, with four option years to extend from October 01, 2008 through October 01, 2012. REQUIREMENTS: The NF/SG Veterans Health System, VA Medical Center (VAMC) 619 South Marion Avenue, Lake City, FL 32025 (hereafter known as the VAMC), has a requirement for the pick-up and disposal of bio-hazardous medical waste. The Contractor shall provide a bio-hazardous medical waste container with lockable doors and appropriately marked "Bio-Hazardous Waste" to be switched out twice per week, unless needed sooner, via 24 hour notice. The Contractor shall ensure that an empty, cleaned and sterilized container is dropped off each time a pickup is made. The anticipated volume is 35,000 per month, requiring a 38 cubic yard container. The Contractor shall obtain the proper permits required for this effort and ensure that the bio-hazardous waste shall be properly transported to an approved treatment facility. A tracking document/manifest shall be provided to Lake City Division VAMC describing the transported material. Contractor shall provide a monthly report of all pickups and disposals, including total weight and number of containers upon submission of an invoice. Quotes will be due Friday, February 29, 2008 by 4:30 pm EST. Only Electronic quotes will be accepted. Quotes shall be emailed to rachel.griner@va.gov. PRICING AND LINE ITEMS: Quotes shall be provided on a Price-Per-Pound basis. Poundage is estimated; exact amounts cannot be determined or guaranteed. Price shall include a price per pound amount, times 35,000 pounds per month. Offerors shall provide a total amount for the base year and all option years, and a grand total for the entire term of the contract. Service Contract Act Wage Determination No. 2005-2115, Rev. 4, dated 5/29/07 shall be included in this contract with labor rates that need to be considered when determining quotes. CONTRACT PROVISIONS AND CLAUSES: Contract clauses and provisions cited in this solicitation may be accessed electronically at these addresses: http://www.arnet.gov/far. In accordance with FAR 52.204-7 - Central Contractor Registration (CCR) (Jul 2006); Contractors are required to register in the CCR database at http://www.ccr.gov, prior to award of any contract. As a Service Disabled, Veteran Owned Business, the offeror are also required to be registered in the VetBiz.gov website at http://www.vetbiz.gov. Failure to register in the aforementioned databases will result in a non-responsive determination. The following clauses apply to this acquisition: FAR 52.212-1, Instructions to Offerors - Commercial Items (Nov 2007), FAR 52.212-4, Contract Terms and Conditions-Commercial Items (Feb 2007), and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (Dec 2007). FAR 52.212-2, Evaluation-Commercial Items (Jan 1999) also applies and evaluation criteria will be based on price, technical capabilities, and past performance. Vendors shall provide at least two references of past performance with their quote, to include name and phone number or email address. Evaluation of quotes may be made with or without discussions with the offeror(s). Award will be based on overall best value to the Government. Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items with its offeror. OFFERS: All responsible sources that meet the VAMC requirements may respond to this solicitation by submitting a quote that provides; (1) All the documentation (as listed above), (2) Pricing, and (3) Response to Evaluation Factors. Any offer that does not meet the solicitation requirements as deemed by the Contracting Officer may be rejected as non-responsive.
