Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation VA-259-07-PR-0220 is issued as a request for proposal (RFP). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-23. The North American Industry Classification System (NAICS) number is 423450, which has a small business size standard of 100 employees. REQUESTERS MUST STATE THEIR BUSINESS SIZE AND SOCIOECONOMIC STATUS ON EACH REQUEST. The VA Montana Healthcare System intents to lease one fully equipped 1500 gallon oxygen tank with delivery of Medical Grade Liquid Bulk Oxygen. The contractor will provide all supervision, labor, tools, equipment, supplies, and transportation to deliver and install a leased 1500gal oxygen tank. Medical Grade Liquid Bulk Oxygen will be delivered on an as needed basis. The average monthly usage is 912gal/104,880cu ft with an annual usage of 11,400gal/1,312,140cu ft. Location of requirement is the VA Medical Center at Fort Harrison, Montana. Pursuant to 38 U.S.C 8127(i), set-aside preferences for awarding this contract will be given to small business concerns and shall be applied in the following order of priority: (1) SDVOSB, (2) VOSB, (3) Contracts awarded pursuant to: Section 8(a) of the Small Business Act (15 USC637(a)); or Section 31 of such Small Business Act (15 USC 657(a)) (HUBZone); and (4) Contracts awarded pursuant to any other small business contracting preference. Evaluation will be based on price, contractor technical capability, and past performance. Proposals should include all inclusive pricing for a 5 year period. Interested parties are responsible for accessing and downloading documents and forms from that site. The following provisions and clauses are hereby incorporated by reference: FAR 52.212-1 Instructions to Offerors - Commercial Items, FAR 52.212-3 Offeror Representations and Certifications - Commercial Items, a completed copy of FAR 52.212-3 must be submitted with all offers, FAR 52.212-4 Contract Terms and Conditions - Commercial Items, FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items including the following additional FAR clauses referenced in FAR 52.212-5, subparagraphs (b) (5), (15), (16), (18), (19), (21), and (36). The following additional clauses apply: CCR Clause: Attention: You must be registered in CCR to be considered for award. FAR 52.204-7, Central Contractor Registration (B)(2) the offeror shall provide its DUNS or, if applicable, it's DUNS+4 number with its offer, which will be used by the contracting officer to verify that the offeror is registered in the CCR database. (C) Lack of registration in the CCR database will make an offeror ineligible for award.
