Description
TESTING LABORATORIES FOR CHEMICAL AND PHYSICAL ANALYSES--SOURCES SOUGHT SYNOPSIS. This announcement is to solicit interest for the establishment of Blanket Purchase Agreements (BPAs) with small businesses. The NAICS solicited is 541380  Testing La boratories. The small business size standard for NAICS 541380 is $11.0 million. The BPAs duration will be for a period of one (1) year and may be extended in one (1) year increments, not to exceed 5 years. The tests to be performed and the test methods to be used are as shown on Table 1 (Attachment A). The establishment of BPAs does not imply any agreement by the Government to place future contracts or orders with the BPA holders. The Government makes no representation as to the number of calls or act ual amount of work to be ordered. The establishment of BPAs does not justify purchasing from only one source. The Government intends to award multiple BPAs for this requirement. Price reasonableness will be determined by competing BPA calls among BPA ho lders. If, for a particular purchase greater than the micro-purchase threshold, there is an insufficient number of BPAs to ensure maximum practicable competition, the Contracting Officer shall solicit quotations from other sources and make the purchase as appropriate. Orders may be placed for USAERDC, Environmental Chemistry Branch, Vicksburg, MS. BPA holders will be selected based on successful past performance of same/similar services, and business experience. Responses to this announcement must inclu de past performance information and business experience to include contact names, addresses, phone numbers, contract numbers, as well as a description of the services provided. Past Performance Questionnaire  For all contacts identified, a Past Performa nce Questionnaire must be completed and submitted. Past Performance Questionnaire is available upon request by email. The prospective contractor (Offeror) shall complete Part 1 of the Past Performance Questionnaire and e-mail/mail the questionn aire to at least two (2) points of contact (POCs) per contract including the Contracting Activity and the Technical Representative responsible for the past/current contract. All POCs shall be instructed to electronically complete Part II of the questionnaire and e-mail to Cynthia Sims, Contract Specialist, at the following e-mail address: Cynthia.L.Sims@mvk02.usace.army.mil. The Offeror shall also e-mail a reference list of all the POCs who were sent a questionnaire. The Government should receive this list no later than close of business on 28 November 2007. The POC list shall be submitted in Word for Windows Table Format to include the following fields: Offeror company name, Contract Number, Customers Name (that received your services), POC last name, first name, and POC Title. The responsibility to send and track the completion of the Questionnaire rests solely with the Offeror and shall not be delegated to any subcontractors, team contractors, or joint venture partners. It is the Offeror's responsibility to contact each reference to: 1) obtain authorization to release the information identified in the past performance submission, and 2) provide authorization to the references to release performance information to the Gove rnment. The completed questionnaires may be submitted by mail, electronic mail (preferred) or facsimile transmission.
