Description

Contractor to provide Oral Surgery Services at the VA Black Hills Health Care System, Hot Springs, SD. Services shall be provided on site, bi-monthly and billed on a procedure basis. Contract shall be for a base year plus four additional option years. Contract commencement shall be January 1, 2008. Requests for the solicitation package must be in writing and addressed to the attention of the Contracting Officer, (Log-Con-HS), VA Black Hills Health Care System, Hot Springs, SD 57747. Faxed requests for the solicitation package may also be sent to 605-745-2831. to the attention of the Contracting Officer. Solicitation package will be available on or about November 21, 2007. Offers are due on December 14, 2007 by close of business 4:30pm local time. Applicable NAICS code is 632320, Dentist Offices. All responsible sources may submit a bid which shall be considered.
