Furnish 1.0 full time equivalent (FTE) Urologist to perform the requirements of this contract. 720 hrs. $ $_____________ STATEMENT OF WORK FOR UROLOGY PHYSICIAN SERVICES DESCRIPTION OF SERVICES: 1. The Contractor shall provide a board certified urologist licensed to perform the requirements of this contract. All work will be performed at the VA Medical and Regional Office Center, 215 North Main Street, White River Junction (WRJ), Vermont. The services to be performed by the contractor will be performed within the VA standards policies and procedures and the regulations of the medical staff by-laws of this VA facility. Work will be in accordance with the VA Standards (Chapter 26). Contractor personnel shall meet all credentialling requirements of the WRJ, VT, VA Medical and Regional Office Center. Personnel assigned by the contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. The qualifications of such personnel shall be subject to review by the VA Medical Director and approval by the VA Facility Director. All work will be under the direction of the VA WRJ Chief of Staff and the Chief, Surgical Service. 2. This task order will be performed based on an hourly rate for 1 FTE. Definition of 1 FTE: a 1 full time equivalent (FTE) is defined as a contractor's employee working eight (8) hours per day, five (5) days per week. A contractor's employee must be present at the VA White River Jct., VT and must be actually performing the required services for the period specified in the contract or the contract cost will be decreased accordingly during each billing cycle. A record keeping system of contractor hours worked will be established. Lunch periods and/or leave will be deducted from the contractor work hours. The payments for any leave, including sick leave, holiday or vacation time, are the responsibility of the Contractor. 3. The VA will award this task order based on a best value decision of the proposed candidates. Candidates will be rated on their : 1) Availability 2) Quality of professional training, including medical school(s) and residency. 3) Board certification in urology. 4) Professional work experience - both duration and quality of positions held. 5) Geographic proximity. 6) Length of possible service term. 4. Medical Liability Insurance will be at the rates identified in the Federal Supply Schedule. 5. Emergency call is not required under this agreement. 6. Services to be performed shall include, but are not limited to: a. Urology Services (1) Provide management of the full range of Urology medical services. The service provider will provide inpatient, outpatient, and surgical intervention to beneficiaries of the WRJ VA. Services will include patient assessment, reassessment, treatment protocols, patient management, and surgical intervention as necessary. Patient management will consist of that measure of continuity of care necessary to effect quality care and good outcomes for the patient. Procedures that may be required under this contract include, but are not limited to: Flex Cystoscopy, Rigid Cystoscopy, TURBT, TURP, Urography, Radical Orchiectomy, Vasectomy, Dilations, Stone Basketing procedures, Hydrocele/Spermatocelectomy, Circumcision, TUI BNC, Suprapubic Cystostomy, Suprapublic Prostatectomy, Urodynamics, Stent Insertion. (2) Urologist shall maintain the existing ongoing program for overseeing the therapeutic programs of inpatients and outpatients. Follow a panel of urology patients in the urology clinic and supervise and teach a urology physician assistant and oversee the work of a urology senior resident. The current urology schedule is Monday all day surgery; three half-day outpatient clinics per week; the remaining three half-days are for administration, patient consults, emergency room coverage; Friday staffing of urodynamics, outpatient Cystoscopy and oversight of residents. (3) Attend Tumor Board meetings and participate in the multidisciplinary care planning of urology patients. The physician will be required to see inpatient urology consults each day, and to see emergency consults as required. (4) Provide urology physician services for consultation as required. Consultations shall be staffed within 24 hours, with appropriate documentation in the patient record. The service provider will triage outpatient consults based on the urgent need of the patient. Individual will be responsible for co-signing all patient notes of the physicians assistant and the senior resident. (5) Participate in the education, teaching, and supervision of a physician's assistant and a urology senior resident in the diagnosis and treatment of VA Medical Center patients including assessing, evaluating, planning care, and performing surgical procedures. Assist in the teaching of interns, surgical residents, medical students, and nursing staff in the care and treatment of a complex nature that result in a multidisciplinary approach to patient care. 7. Special Contract Requirements a. The White River Jct., VA maintains a computerized medical records system, individuals performing work under this agreement must be computer literate and be proficient in keyboard and word processing skills. All personnel providing services under this contract shall speak and write English proficiently. b. Contract personnel shall participate in the education program conducted by the VA of the Medical Staff necessary to assist the Medical Center in maintaining overall compliance with accrediting requirements and shall perform such other reasonable teaching functions within the Medical Center as may be requested. c. Contractor shall provide professional direction to residents rotating to the VA Medical Center. Contractor shall abide by the regulations of this Medical Center and Veterans Health Administration governing the supervision of residents (Chapter 26), including documentation requirements. d. Service provider is required to be BLS certified and credentialed in conscious sedation. e. There is no employer-employee relationship between the VA and the contractor or the contractor's employee(s). The Government may evaluate the quality of professional and administrative services provided, but retains no control over the medical, professional aspects of services rendered (e.g., professional judgments, diagnosis for specific medical treatments). The level of service shall be consistent with the current requirements to meet and maintain Joint Commission Accreditation of Healthcare Organizations (JCAHO) standards. f. Medical Records - Clinical or other medical records of VA beneficiaries treated by the Contractor will remain at the VA. g. Key Personnel And Temporary Emergency Substitutions: (1) During the first 90 days of performance, the Contractor shall make NO substitutions of key personnel unless the substitution is necessitated by illness, death, or termination of employment. The Contractor shall notify the Contracting Officer, in writing, within 15 calendar days after the occurrence of any of these events and provide the information required by paragraph (c) below. After the initial 90-day period of the contract, the Contractor shall submit the information required by paragraph (c) to the Contracting Officer at least 15 days prior to making any permanent substitutions. (2) The Contractor shall provide a detailed explanation of the circumstances necessitating the proposed substitutions, complete resumes for the proposed substitutes, and any additional information requested by the Contracting Officer. Proposed substitutes shall have comparable qualifications to those of the persons being replaced. The Contracting Officer will notify the Contractor within 15 calendar days after receipt of all required information of the decision on the proposed substitutes. The contract will be modified to reflect any approved changes of key personnel. (3) For temporary substitutions where the key person will not be reporting to work for three (3) consecutive work days or more, the Contractor will provide a qualified replacement for the key person. This substitute shall have comparable qualifications to the key person. Any period exceeding two weeks will require the procedure stated in paragraph (b) above. h. All work must be performed in accordance with JCAHO agency standards. A copy of these standards may be obtained from the Joint Commission on Accreditation of Healthcare Organizations, One Renaissance Blvd., Oakbrook Terrace, IL 60181. (1) The Contractor will be responsible to ensure that Contractor employees providing work on this contract are fully trained and completely competent to perform the required work. All providers shall provide evidence of current immunizations. Contractor must provide the following documentation of all personnel performing services under this agreement: Documentation of PPD testing, documentation of current tetanus toxoid, immunity to rubeola, rubella, varicella, documentation of current hepatitis B vaccination. (2) The Contractor is required to maintain records that document competence/performance level of Contractor employees working on this contract in accordance with JCAHO and other regulatory body requirements. The Contractor will provide a current copy of the competence assessment checklist and annual performance evaluation to the Contracting Officer's Technical Representative (COTR) for each Contractor employee working on this contract. (3) When changes in Contractor personnel are approved in accordance with the "Key Personnel" clause of the contract, the Contractor must provide the providers name, a current address and telephone number, Social Security Number, list of references, list of all training, evidence of an unrestricted license, the current competence assessment, and current performance evaluation that supports #1 above. (4) The contractor is required to develop and maintain the following documents for each Contractor employee working on the contract: credentials and qualifications for the job; a current competence assessment checklist (an assessment of knowledge, skills, abilities and behaviors required to perform a job correctly and skillfully; includes knowledge and skills required to provide care for certain patient populations, as appropriate.); a current performance evaluation supporting ability of the Contractor employee to successfully perform the work required in this solicitation; and listing of relevant continuing education for the last two years. The Contractor will provide current copies of these records at the time of contract award and annually on the anniversary date of contract award to the VA COTR, or upon request, for each Contractor employee working on the contract. 8. Term of Contract This contract is effective from December 3, 2007 through February 8, 2008 with one 3-month option to renew. This contract may be amended at the option of the VA. The total length of this contract shall not exceed six months. This contract is subject to the availability of VA funds. No service shall be performed by the contractor after May 30, 2008, until the contracting officer authorizes such services in writing. 9. Work Hours Work Hours: The services covered by this contract shall be furnished by the contractor as defined herein. The contractor will not be required, except in case of emergency or as otherwise indicated in this agreement, to furnish such services on a national holiday or during off duty hours as described below which have the following meanings: a. Work Hours - Monday 7:00 a.m. - 3:30 p.m; Tuesday thru Friday 8:30 a.m. - 5:00 p.m. b. National Holidays: The ten (10) holidays observed by the Federal Government are as follows: New Year's Day Martin Luther King's Birthday Washington's Birthday Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Christmas Any other day specifically declared by the President of the United States to be a national holiday. c. Off Duty Hours: Monday 3:30 p.m. - 8:30 a.m., Tuesday thru Friday 5:00 p.m. - 8:30 a.m, and all Saturdays and Sundays. 10. Personnel Policy The contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the contractor shall provide the following for these personnel: worker's compensation professional liability insurance health examinations income tax withholding social security payments The parties agree that such personnel shall not be considered VA employees for any purpose and shall be considered employees of the contractor. 11. Payment a. The VA agrees to reimburse the Contractor on a unit basis for resources furnished at the prices listed in the Schedule of Supplies/Services portion of this contract. b. Payment of sums due will be paid monthly upon submission (by the contractor) of a properly prepared invoice and receipt in the appropriate VA office. The VA will review the records for agreement with the VA records and verify that the coverage provided for the month is in accordance with the contract. Payment will be authorized by the VA upon verification by the VA of the services billed. c. Billings rendered by the contractor to the VA for services furnished a VA beneficiary under the terms of this contract shall be billings in full. Neither the beneficiary, his/her insurer nor any other third party shall be billed. d. In the event that coverage is not provided in accordance with this contract, the payment to the contractor shall be reduced in direct ratio to the reduction in coverage. Any additional costs incurred by the VA as a result of obtaining coverage from a source other than the contractor, due to the contractor's failure to provide adequate coverage, will be assessed against the contractor. 12. Recordkeeping A recordkeeping system, by use of time and attendance logs, will be established at this VA to demonstrate that services called for under the contract have been received by the VA. These logs will be maintained by the Surgical Service Secretary . Contractor's service provider shall sign in and out with the Surgical Service Secretary when coming on duty and when leaving duty. In the event that work is performed during off duty hours, sign in will be accomplished with the Administrative Officer of the Day (AOD). 13. Termination The Government may terminate this contract at any time upon at least 30 days' written notice by the Contracting Officer to the contractor. The contractor, with the written consent of the Contracting Officer, may terminate this contract upon at least 30 days' written notice to the Contracting Officer. 14. Contractor Personnel Security Requirements All contractor employees who require access to the Department of Veterans Affairs' computer systems shall be the subject of a background investigation and must receive a favorable adjudication from the VA Office of Security and Law Enforcement prior to contract performance. This requirement is applicable to all subcontractor personnel requiring the same access. If the investigation is not completed prior to the start date of the contract, the contractor will be responsible for the actions of those individuals they provide to perform work for VA. a. Position Sensitivity - The position sensitivity has been designated as Low Risk. b. Background Investigation - the level of background investigation commensurate with the required level of access is Minimum Background Investigation. c. Contractor Responsibilities 1) The contractor shall bear the expense of obtaining background investigations. If the investigation is conducted by the Office of Personnel Management (OPM), the contractor shall reimburse VA within 90 days. 2) The contractor shall prescreen all personnel requiring access to the computer systems to ensure they maintain a U.S. citizenship and are able to read, write, speak and understand the English language. 3) The contractor shall submit or have their employees submit the following required forms to the VA Office of Security and Law Enforcement within 30 days of receipt: (i) Standard Form 85P, Questionnaire for Public Trust Positions (ii) Standard Form 85P-S, Supplemental Questionnaire for Selected Positions. (iii) FD 258, U.S. Department of Justice Fingerprint Applicant Chart (iv) VA Form 0710, authority for Release of Information form (v) Optional Form 305, Declaration for Federal Employment (vi) Optional Form 512, Optional Application for Federal Employment d. The contractor, when notified of an unfavorable determination by the Government, shall withdraw the employee from consideration from working the contract. e. Failure to comply with the contractor personnel security requirements may result in termination of the contract for default.
