Description
This is a combined synopsis/solicitation for the contractor to provide a portable Magnetic Resonance Imaging scanner at the Lebanon VAMC, Lebanon PA 17042. There is an additional tasking of rigging the trailer and unit to its attachment location at the Lebanon VAMC. Attachment area at Lebanon VAMC has a canopy (roof) that is 13'2" high. Dedicated 480 volt 3 phase power and networking capabilities are available at this location. Trailer units shall not have roof access service panels. There is also an option to include a canopied walkway if unit cannot be rigged into place under existing canopy area; this is to be considered as a last resort. Offerors are required to comply with all terms and conditions of the solicitation. STATEMENT OF WORK: Vendors are requested to submit a proposal for a mobile MRI scanner and housing trailer at the Lebanon VAMC. Proposal will be for the unit to remain in-place for duration of contract and contract options as exercised by the Lebanon VAMC. Contractor's Modality Engineer will establish network and printer connectivity at each site in coordination with local IT Staff. Equipment will be capable of sending images to a minimum of 3 destinations (printer, Pac_s, Hospital RIS System, etc) CD Burner on van. Lebanon VAMC will provide all staffing for portable MRI unit to include: MRI Technologist(s), Radiologists, and support personnel; 4 days of successful applications training must be provided by vendor. Successful bidder will provide routine and emergency maintenance to unit so as not to conflict with scheduling of patients as feasible; bidder must consider repair after normal working hours. RECORDS AND CONFIDENTIALITY Records created in the course of treating VA patients under this agreement are the property of the VA and shall not be accessed, released, transferred or destroyed except in accordance with applicable federal laws and regulations. At the conclusion of this contract the hard drive supporting the MRI scanner must have all patients demographic and scan information removed; hard drive must be sanitized by Lebanon VAMC IT staff. STATEMENT OF NON-EXCLUSIVITY The requirements set forth in this agreement shall be considered non-exclusive in nature as they relate to the VA's ability to also utilize other Contractors for the provision of services stated herein. This shall not however reduce or modify any other obligation established herein or mitigate either party's responsibility to comply with any and all Federal, State and or Local law regulating the provision of health care services in State of Pennsylvania and/or other applicable accreditation standards. INVOICES As full and complete compensation for all services provided by the Contractor under this agreement, the VA agrees to make payment on a timely basis for services rendered, upon receipt of properly prepared invoices submitted by the Contractor as outlined. VA agrees to pay the rates specified by this agreement and any rate otherwise agreed upon through modification of this agreement. Payment shall be made by electronic transfer of funds. All payments will be made monthly in arrears, after receipt of a properly executed invoice. Contractor shall submit an itemized monthly statement. All invoices shall contain the following information: a. Contract Number b. Obligation Number. TECHNICAL QUESTIONS Offerors should submit all technical questions regarding this solicitation to the Contracting Officer in writing; Verbal questions will not be addressed. All responses to questions, which may affect offers, will be incorporated into a written amendment to the solicitation. Note: the Government is not obligated to request any additional information from an offeror who does not provide sufficiently detailed information so that an accurate evaluation can be made of the proposal. This solicitation is issued as an unrestricted request for quotes (RFQ). The NAICS code is 532120 with size standard $23.5 Million. The contract shall be for a period of three (3) months with the option to extend the contract one month at a time for up to a total of nine (9) months. Rigging shall be included as part of the monthly price. The Government will award a firm fixed price contract resulting from this solicitation to the responsible offeror whose offer provides the best value to the Government. In accordance with 52.212-2 (Jan 1999) the following factors will be considered during the evaluation process. Offers will be evaluated based on: (1) Past performance: Provide three current references in which your company has performed the same or similar type of work as described in this solicitation. Each reference must represent work that was performed within the past three years. Include company name, address, telephone number and point of contact. (2) Technical: Provide complete documentation showing the dimensions and specifications of the mobile trailer, power and plumbing connections that will be used or required i.e. width, height length, voltage, configuration, frequency, regulations etc. (3) Price: Provide a written price quotation covering the first three (3) month period plus a separate price for each consecutive month up to nine (9) months. Evaluation: Price is more important than Technical and past performance factors, when combined. Technical and past performance are equal in weight. How to apply: The Contractor shall include the following as a total solicitation package: Written price quotation for the three (3) month period as well as the nine (9) month periods, responses to all evaluation factors (past performance and technical), a completed copy of FAR 52.212-3 or complete the annual representations and certifications electronically at http://orca.bpn.gov; and any literature and brochures with specific information regarding the mobile unit to be provided.
