Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Request for Quotes (RFQs) being requested and a written solicitation will not be issued. This solicitation, VA-249-08-RQ-0003 is issued as a Request for Quote. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-19. The North American Industry Classification System (NAICS) number is 423450 - Medical, Dental, and Hospital Equipment and Supplies Merchant Wholesellers, which has a small business size standard of 100 employees. The Veteran Administration Medical Center (VAMC) located at 1101 Veterans Drive Lexington, KY 40502 intends to procure STAXI nestable transport chair systems. The transport chairs must able to carry up 450 weight capacity, shall have from seat to floor 21 inches, back height 41 inches. The casters 5 inch with 5/8 inch stem, urethane, sealed bearings and non-marring. The foot rests must be stainless steel (non-removable), lifts for comfort. The arm rests must be steel oval tubing, non-porous sleeve and lift for easy transfer. The wheels must be 2 1/8 composite (8 inch diameter - non-marr). The nestable transport chair purchase is for the following line items: (1) Nestable Transport chair (COLOR - RED) STAXI # ST017 (2) Retractable seat belt that is expandable STAXI# ST092 (3) IV pole holder, must be a steel fixture, capable of fitting both 5/8 inch and 7/8 inch poles STAXI #ST070 (4) Oxygen tank holder, 4.5 inch diameter STAXI # ST075 (5) Cushion - Antimicrobial treated, easy clean finish (COLOR - BLACK). STAXI # ST059 A quantity for each of the five (5) line items is 100 each. The warranty on these items is three (3) years for all labor and three (3) for all parts. Delivery is 10 days after receipt of the purchase order. Product substitution will not be acceptable. FOB will be destination. Clauses 52.252-1 Solicitation Provisions Incorporated by Reference and 52.252-2 Clauses Incorporated by Reference are included in this synopsis/solicitation. Interested parties are responsible for accessing and downloading documents and forms from that site. The following provisions and clauses are hereby incorporated by reference: FAR 52.212-1, Instructions to Offerors, Commercial Items FAR 52.212-2, Evaluation - Commercial Items. The Government will award a purchase order resulting from this solicitation that will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers: 1) Technically acceptable and 2) Price. The award will be made on the basis of the lowest evaluated price of proposals that are technically acceptable. Technically acceptable will be evaluated based on the requested part number/description for each of the five (5) line items listed above. Offerors shall submit a quote that details part number by line item, by unit price per line item, the extended price for 100 units for each line item, and grand total price. Warranty information is also to be provided. Price will be evaluated to determine its fairness, completeness and reasonableness as it relates to the items offered. As prescribed in FAR Part 42.15, the VA evaluates contractor past performance on all contracts that exceed $100,000 and shares those evaluations with other Federal Government agencies. The FAR requires that the contractor be provided an opportunity to comment on past performance evaluations prior to each report closing. To fulfill this requirement, VA uses an online database, the Contractor Performance System (CPS, which is maintained by the National Institutes of Health (NIH). The CPS database is shared with the Past Performance Information Retrieval System (PPIRS) database, which is available to all federal agencies.
