- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, FA2517-07-T-6098, is being issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-20. This procurement is a 100% Small Business set-aside IAW FAR 19.502-2 (a). The NAICS code is 811490 and the size standard is $6.5 million. The following commercial item is requested in this solicitation; STATEMENT OF WORK (SOW) PREVENTATIVE MAINTENANCE OF CARDIOVASCULAR AND WEIGHT EQUIPMENT FOR PETERSON AFB AND CHEYENNE MOUNTAIN AFS, CO 20 Sept 2007 1. Description of Work a. Scope: The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items necessary to inspect, repair and maintain in operational condition cardiovascular and weight equipment in the Peterson AFB and Cheyenne Mountain AFS (CMAFS) Fitness Centers in accordance with (IAW) this statement of work. In total, there are 3 Fitness Centers, one on Peterson AFB, one outside of CMAFS and one inside CMAFS. b. The Fitness Center Director or Operations Director must approve all repairs prior to beginning work. The Fitness Center does not have to approve routine and general preventative maintenance work; however, the contractor must coordinate all preventative maintenance inspections with the Fitness Center Director or designee, which will be performed in accordance with paragraph 7a. c. Contractor shall invoice once per month. Invoice amount shall be firm fixed price as set forth in the schedule at time of award (or changed thru modification) and will be submitted at the end of the month for services renderred. 2. Personnel a. Contract Manager: The individual responsible for the performance of the repair work. If this individual is not available, an Assistant Contract Manager is responsible. The names of all individuals who will be involved in performing work must be provided to the Fitness Center Director or the designee to allow for coordination with Security Forces personnel to gain access to Peterson AFB and CMAFS. If this coordination does not occur at least three days prior to an anticipated visit, there may be a delay in obtaining access passes. b. Employees: The government is authorized to restrict the employment under this contract of any contractor employee or representative contractor employee who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of these facilities or its patrons. c. All individuals who will perform work on equipment must be certified and qualified to service and maintain the equipment identified in this SOW. The contractor must provide documentation to substantiate this. d. Contractor employees shall wear appropriate attire while performing maintenance service. Contractor personnel shall present a clean, neat and professional appearance at all times and be easily recognized as contract employees. 3. Quality Control a. The contractor must ensure that all work performed under this SOW meets the quality standards established in paragraph 7. b. Any deviations to paragraph 7 must be in the scope of the SOW and approved by either the Fitness Center Director or designee before the repair. 4. Hours of Operation a. The contractor must provide a minimum of a 3-day notification of intent to perform work associated with this SOW. This will allow Fitness Center staff to notify patrons and adjust operations if necessary and/or possible. Work needs to be scheduled between the third and twenty-seventh of the month. b. The contractor shall schedule all work to be done on equipment between the hours of 0700 and 1900 Monday through Friday with the understanding that work cannot be accomplished between the hours of 1000 and 1400 at the Peterson AFB Fitness Center due to high usage. c. Work may not be accomplished on federal holidays. These include New Years Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving, and Christmas. If the federal holiday falls on a Saturday or Sunday, the observed day may be a Friday or Monday as directed. 5. Government Furnished Supplies and Services a. A Fitness Center representative must be on site for all scheduled service calls. b. The Fitness Center Director or designee is required to sign all service reports, verifying the satisfactory completion of the service(s) performed. 6. Contractor Furnished Parts and Materials a. Replacement Parts: The contractor shall provide all parts costing $250 or less. For parts exceeding $250, the Fitness Center Director or designee will purchase the part and make it available to the contractor to use in the repair. b. All replacement parts will be new factory warranty parts, unless replacement manufacturer rebuilt parts are approved by the Fitness Center Director or designee. All parts replaced remain property of the Fitness Center and must be made available at the completion of the repair. 7. Specific Tasks a. The contractor will provide scheduled preventative maintenance inspections (PMIs) in accordance with the manufacturer specifications. Preventative maintenance shall be accomplished within the prescribed timeframe as recommended by the equipment manufacturer. The contractor shall develop the PMI schedule so that service technicians will be on-site performing inspections/maintenance at least one time per month. This consists of the care and servicing of the equipment for the purpose of retaining the equipment in a serviceable condition IAW the manufacturers maintenance specifications. The specifications are listed in the manufacturers technical literature. This inspection includes, but may not be limited to, checking for proper operations, detection, and correction of incipient failures, either before they occur or before they develop into major problems. This includes lubrication, adjustments, calibration, functional checks, and interior cleaning of all equipment and replacement of all damaged or deteriorated parts. b. Service not included in this contract includes: Replacement of electrical motors burned out by electrical current fluctuation, negligence, abuse, or misuse, as determined by the contracting officer. Expendable supplies and accessories greater than $250. Electrical work exterior to the machine. Repairs and parts necessitated by accident, fire, abuse, or misuse by government personnel as determined by the contracting officer. On-call and/or emergency repair service. c. Before replacing parts on any piece of equipment, the contractor will provide the Fitness Center Director or the designee a list of parts required, as well as the cost of the parts. No part will be replaced without prior approval. d. All replacement parts required for maintaining equipment will be ordered within 72 hours from time of approval. e. Upon delivery of the replacement part to the Fitness Center and contractor notification of arrival, the piece of equipment shall be repaired and operational within 48 hours. f. The contractor?s service technician will prepare a service report for each visit and obtain a signature from the Fitness Center Director or designee to certify that the services were completed and accepted. The report will include work accomplished, work pending, and any other recommendations in regards to the equipment. 8. Points of Contact a. Any matter concerning changes to the scope, prices, terms, or conditions of this contract shall be referred to the contracting officer. 10. Attachments a. Attachment A: List(s) of current equipment. These lists are subject to change based on replacement of existing equipment and addition of new equipment. When this occurs, the Fitness Center Director or designee will provide an updated equipment list to the contracting officer for incorporation thru modification. The following provisions and clauses apply to this acquisition: The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items. FAR 52.212-2(a) Quote will be based on price. Contract award will be for a total of six (6) years. The provision at FAR 52.212-3, Offeror Representations and Certification.
