Description
The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 3), James J. Peters VA Medical Center, Bronx New York, Network Acquisitions and Logistics program is currently seeking a firm who can provide "REPAIR AND REFURBISH SURGICAL INSTRUMENT" PM - Preventive maintenance/scheduled repair, refurbishment and Inspection for Surgical Instruments - medical instruments used in surgery. This includes but is not limited to the following: curets, curettes, hemostats, scissors, surgical knifes, etc. Services which are periodic in nature and are required to maintain the equipment in such condition that it may be operated in accordance with its intended design and functional capacity with minimal incidence of malfunction or inoperative conditions. Contract service shall ensure that the equipment functions in conformance with the latest published edition of NFPA-99, OSHA, CDRH, and [include performance standards and specifications used when the JAMES J. PETERS VAMC REPAIR & REFURBISHMENT OF SURGICAL INSTRUMENTS was procured by VA National Acquisition Center and any upgrades/updates]. The contractor shall maintain the equipment in accordance with the Conformance Standards Section, by furnishing all necessary labor, equipment, tools, materials, repair service and parts for service (excluding consumables) in accordance with the terms, conditions and schedule of agreement. The contractor will provide repair service which may consist of calibration, cleaning, oiling, adjusting, replacing parts, and maintaining the equipment, including all intervening calls necessary between regular services and calibrations. Upon completion of the Preventive maintenance/scheduled repair and refurbishment and electrical safety inspection, the FSE must tag the equipment with the appropriate labels used by this facility. The FSE must fill-in label sections for: date inspected, date next due, and initials of FSE. Labels will be provided to the FSE when reporting (signing-in) to Biomedical Engineering. The firm will be responsible for providing all management, supervision, labor, materials, supplies and equipment, and shall plan, schedule, coordinate and assure effective performance of maintaining and operating the modification of this vehicle Offers will be considered only from offertory who are established in the business called for and whom in the judgment of the Contracting Officer (CO) are financially responsible and able to show evidence of their reliability, ability, experience, equipment, facilities and personnel directly employed or supervised by them to render prompt and satisfactory service.. Requested services must meet the specifications for James J. Peters VA Medical Center, 130 West Kingsbridge Road Bronx, New York 10468. Applicants must have at least FIVE YEAR OF SPECIALIZED EXPERIENCE. Preferred if the applicant had provided same or similar services to other VA Medical Center throughout the United States. The applicant must also provide "REFERENCES" for providing same or similar products. Bronx VA Medical Center is looking for a firm who can provide same or equal brand (Product). All contractors must list of all description of modifications to be provided Firms desiring consideration shall email directly to the Contracting Officer referencing the following in the subject field, "REPAIR AND REFURBISH SURGICAL INSTRUMENT", then contain the following information within the body of the e-mail name of firm, complete address, phone & fax number of contact. When a small business receives an award as prime contractor as a result of a small business set-aside solicitation, the small business prime will be required to comply with FAR 52.219-14, Limitations on subcontracting, which states that at least 50 percent of the cost of contract performance incurred for personnel shall be expended for employees of the concern. The contract requires that the selected firm have an on-line access to E-mail via the Internet for routine exchange of correspondence.
