- This is a Commercial Item solicitation using FAR Part 12 and 15. The VA Salt Lake City Health Care System, Salt Lake City, Utah is seeking proposals for a Community Based Outpatient Clinic (CBOC) to cover primary care services in Duchesne, Uintah, and Daggett counties in the state of Utah. The contractor shall provide for the delivery and management of continuous primary and preventative medical care for eligible VA patients. The contractor shall provide health care clinical providers, medical facilities, medical equipment, supplies, laboratory and imaging services, pharmacy services and all administrative services. Services will be at a capitated rate and will be all inclusive. The period of performance will be for a base year from January 1, 2008 through September 30, 2008 with four, one-year options to extend for a total of five years.
