- Correction: This action is amended to replace the Statement of Work with the following: STATEMENT OF WORK: The contractor will provide outpatient dermatology consultation and procedures as needed. The contractor will provide written consultation detailing contractor's clinical impression and recommendation for evaluation and/or therapy. PURPOSE AND OBJECTIVE: The purpose of having a contract dermatologist is to provide expert specialty care to patients of the Fort Belknap Health Center which they would not otherwise receive. CONTRACTOR RESPONSIBILTIES: The contractor will provide dermatology services to patients of the Fort Belknap Health Center. The services will include dermatology consultations, patient management, diagnostic and therapeutic procedures for which the contractor must obtain privileges at the Fort Belknap Health Center. The contractor must maintain licenses, CME, specialty status and provide proof as required for credentialing. The contractor is responsible for their own transportation. The contractor must assist in super bill completion. GOVERNMENT RESPONSIBILITIES: The Fort Belknap Health Center will provide space and such other assistance as needed to aid the contractor in the delivery of professional services of high quality. The Fort Belknap Health Center will provide transportation for patients scheduled with contractor. BILLING AND COLLECTION: The Indian Health Service will be responsible for billing and collection of all services provided by the contractor. All other information publicized in the original synopsis is correct.
