Description
Statement of Work for Medical Contract 1. Introduction and Scope of Work: This requirement is needed because the demand for Occupational Therapy services at the VA New Jersey Healthcare System, East Orange and Lyons campus exceeds the current level of staffing required to meet the expectations of both internal and external customers. Attempts to hire an FTE have been unsuccessful. The staff occupational therapist is responsible for delivering Occupational Therapy services, as prescribed by the physician, to all types of illness and disabilities found in the acute care inpatient and outpatient settings at East Orange and the nursing home, outpatient, and mental health settings at Lyons, using evaluations and treatments that promote positive outcomes through goal achievement. The mission of the Physical Medicine and Rehabilitation Service (PM&RS) at the VA New Jersey Healthcare System is to provide comprehensive rehabilitation care to the veteran patient utilizing an interdisciplinary team approach using the state of the art equipment and procedures, in order to return the veteran back to the community in the highest level of independent function, and better quality of life. The primary goal of rehabilitation is to help patients, regain physical, psychological, and social functioning to prepare for self-sufficiency. This includes recovering old skills as well as learning new strategies to accomplish every day tasks. The rehabilitation staff will work with each patient and family to develop individual treatment programs that will maximize each patient's potential and enable him/her to achieve the highest attainable skill in the shortest time possible. 2. Requirements: The minimum requirements for the staff Occupational Therapist are as follows. a) Timeliness requirement. The Staff Occupational Therapist will provide services two (2) to five (5) days per week, between the hours of 8:00 a.m. to 7:00 p.m., as determined by the Chief of Occupational Therapy based on clinic needs. All patients scheduled for each day are required to be treated and all documentation completed by the end of the day treated. b) Reporting requirements. Patient documentation is to be entered into the Computerized Patient Record System (CPRS). All patient appointments are scheduled in DHCP/VISTA. Documentation for daily attendance and monthly statistics are provided by electronic report to the Chief of Occupational Therapy. Patient documentation includes, but is not limited to, order receipts, initial evaluations, re-evaluations, treatment, and discharge notes. c) Documentation requirements. The following documentation is required for employment: Occupational Therapy license, NBCOT certificate, Employment Application, Background Check (Form 85). d) The following documentation is required to fulfill the duties of the staff Occupational Therapist. Initial evaluations include, but are not limited to, diagnosis/problem, assessments, findings, plan, goals and patient education. Progress notes are written for each treatment session in SOAP note format, or other format determined by the Chief of Occupational Therapy, to reflect treatment plans and established goals. Reevaluations reflect present and previous levels of function, identify new goals, and are performed and documented two (2) sessions prior to a scheduled follow-up with the physiatrist. Discharge notes are written when an inpatient is discharged from the hospital or discontinued from therapy and include initial and final levels of function and patient/family education. All documentation in CPRS is completed by the end of shift on the day of treatment. Patient attendance is recorded daily and submitted with monthly statistics no later than noon of the second workday of the new reporting period. e) Facility data. The VA will provide the contractor with the use of the Occupational Therapy clinic including assessment tools and equipment therein and a computer for work-related activities. f) Workload data. Historical workload demonstrates that the contractor will be required to treat a minimum of 8-12 patients per day depending upon case mix. g) Place of performance. The contractor will be employed at the VA New Jersey Healthcare System, East Orange Campus, 385 Tremont Avenue, East Orange, New Jersey and/or Lyons Campus, 151 Knollcroft Road, Lyons, New Jersey. h) Procedure for referral and scheduling. Patients will be referred by consult or written orders. These are distributed by the Chief of Occupational Therapy or his/her designee. Per standard operating procedures of the PM&RS, all patients are scheduled in DHCP for treatment within 24 hours of receipt of therapy orders. Within 24 hours of discharge or discontinuance of therapy, the treating therapist is responsible for deleting any unneeded appointments from DHCP. i) Transfer of patients. The contractor has no authority to transfer patients. In emergency situations, the contractor will notify the Chief of Occupational Therapy or a full-time Occupational Therapist on duty or the Service Coordinator or an attending physiatrist. j) Patient Safety. The current safety policies within the facility include, but are not limited to, Environment of Care and Infection Control. k) Billing. Bills must be submitted to Lynn Ryan, Chief of Occupational Therapy. l) Payments. In order for payments to be authorized the contractor must submit an invoice with dates and hours worked on a weekly basis. m) Penalty for non-performance. Non-performance with the terms of the SOW will result in termination of services. n) Reimbursement method. Reimbursement will be FTE based. o) Educational and research activities. Certification by the National Board of Certification in Occupational Therapy, Inc. (NBCOT) and a license in Occupational Therapy in the State of New Jersey or other state recognized by the Department of Veterans Affairs is a requirement for this position. Completion of mandatory training provided by the VA, includes but is not limited to, Fire and Safety, Infection Control, Disaster Procedures, Information Security Awareness Training, ADR/Mediation, Annual Environment of Care, Compliance Awareness, Employee Patient Relationship, External Error Reporting System, HIPAA Privacy, Infection Control, Information Security, Standards of Ethical Conduct, Sexual Harassment, Skills for Effective Teamwork, Violence in the Workplace, Cultural Competency, Pain Management. p) Following the Occupational Therapy Code of Ethics and Standards of Practice as set forth by the American Occupational Therapy Association, Inc. (AOTA) is required for this position. q) Adhering to all facility, PM&RS, and Occupational Therapy policies and procedures including the completion of all mandatory training is required for this position. r) The staff Occupational Therapist works under the supervision of the Chief of Occupational Therapy. s) All official business and correspondence coming into or going out of the Occupational Therapy Department will flow over the desk of the Chief of Occupational Therapy. t) Demonstrating a high level of mental and emotional tolerance and even temperament when dealing with patients, families, and co-workers, using tact sensitivity, sound judgment and professional attitudes is required for this position. u) Communicating with customers (e.g., veterans, family members, and the general public), co-workers and supervisors in a courteous, tactful, friendly, cooperative manner is required for this position. v) Working with a great deal of independence in planning and carrying out complex treatment routines, adapting treatment procedures and changing or modifying treatment plans, with prior approval from the supervisor, is required for this position. w) Performing check-out procedures of patient encounters through CPRS and Event Capture through DHCP on a daily basis is required for this position. 3. Delivery: The following services should be delivered to the Government. a) Performance of Occupational Therapy initial and reevaluation tests and measurements such as ADL assessments, manual muscle testing, range of motion measurements, etc. b) Provision of age appropriate interventions. c) Provision of ADL and equipment training in deficient areas. The therapist is also expected to train both staff and family members. d) Use of physical agent modalities such as Fluidotherapy, paraffin, hot/cold packs, ultrasound, TENS, and other modalities to achieve relief of pain, as prescribed by the physician. e) Use of special exercises such as self-ranging, desensitization, adjusting for sensory losses, retrograde massage, techniques for UE strengthening, energy conservation, and fine motor coordination. f) Administration of cognitive training involving task-skills that depend on attention building, organizing, categorizing, sequencing, and remembering. g) Fabrication and provision of splints and maintaining the patient in proper positioning to minimize/prevent contractures. h) Instruction of patients, nurses and nursing assistants in the clinic and on the unit in proper positioning of patients to minimize and/or prevent contractures. i) Participation in the in-service training lectures and demonstrations for staff Occupational Therapists, Occupational Therapy trainees, medical residents, health care providers and other allied health professionals. j) Completion of appropriate documentation as set forth in § 2. Requirements. 5. Progress and Compliance: The reports required by the VA from the contractor include, but are not limited to, daily attendance and monthly statistics. Pertinent quality assurance requirements are: scheduling of appointments and completion of documentation and check-out procedures in a timely manner and provision of appropriate treatment as per practice standards. The performance standards are as set forth herein and governed by all professional standards of practice of NBCOT, AOTA, and the Occupational Therapy licensure laws of the State of New Jersey. The contract will be monitored by the Chief of Occupational Therapy. He or she will document contractor attendance and performance by direct observation of the contractor, collection of data from patient charts, review of daily schedule and monthly statistics, and tracking of mandatory training completion.
