- 1.0 FISC Norfolk Philadelphia Contracts Division intends to procure Educational Technology Consultant for Assessment and Professional Development of Instructional Methods, for the Naval Hospital Corps School, Great Lakes, IL. Request for Quotation N00189-07-Q-E690 contemplates a firm fixed price contract. Period of Performance: Estimated to begin 27 August 2007 thru 30 September. This is a combine synopsis/solicitation for commercial items prepared in accordance with Simplified Acquisition Procedures. This announcement constitutes the only solicitation requested; written solicitation will not be issued. This is a Small Business Set-Aside RFQ requirement and it is the responsibility of contractor to be familiar with the applicable clauses and provisions. The clauses can be access in full text at these addresses: www.arnet.gov/far and www.dtic.mil/dfars. The NACIS CODE IS 611710. Objectives: Contractor shall a. Provide an outside assessment of the command utilizing surveys and focus groups, in order to provide research based data on the aptitude, attitudes and behaviors of staff. b. Provide a one day onsite professional development seminar to all staff on Blended Learning strategies and practices. c. Perform an in-depth review of the Naval Hospital Corps School educational systems. d. Provide a written report and oral presentation to Naval Hospital Corps School of the assessment and research results. The intent of this is to provide insight into the Naval Hospital Corps School strengths and weaknesses and recommendations for future direction. 1.1 Scope: Utilize a mixed method (qualitative and quantitative) research design for data collection and analysis. Primary tools should include: a staff survey developed by the contractor using NHCS leadership/stakeholder input and staff focus groups. The contractor will be required to provide an in depth review of instructional materials and delivery methods. The data collection should begin upon contract award to be followed by a written report, presentation of results, and the professional development seminar, to be completed by the end of September 2007. 1.2 Contract Type: Firm Fixed Price 1.3 Place of Performance/Hours of Operation. The analysis and data collection phase are to be performed off-site to limit intrusion of the instructional schedule and personnel and limit travel-related expenses; this phase shall not exceed 10 business days. The oral brief, written report and educational seminar will be delivered during the course of one day on-site at NHCS which is located in Great Lakes, IL. The specific date will be coordinated by the contractor with the NHCS POC. The seminar must be conducted on a day between Monday through Friday from 7am to 4pm central standard time. 1.4 Period of Performance. Off-site analysis and data collection may be performed after the date of award but shall take place within thirty days of the on-site visit. The one day on-site shall is estimated to take place between August 27,2007 and September 30, 2007. 1.5 Privacy Act. Work on this project may require that personnel have access to Privacy Information. Personnel shall adhere to the Privacy Act, Title t of the U.S. Code, Section 552a and applicable agency rules and regulations. 2.0 Point of Contact. Todd A. The Government will provide the following: Conference space, computer equipment, instructional materials, telephone and reproduction facilities and office supplies as required. 4.0 Contractor Furnished Items. Any and all materials required to perform assessments, presentation materials, and handouts. 5.0 Specific Requirements. 5.1 Task Description. The contractor shall: Off-Site 1. Interview command leadership and stakeholders in an effort to identify command priorities and assist in creation of an instructor survey tool. 2. Survey staff will tool developed and conduct focus groups. These can be accomplished in an online web-based format either synchronous or asynchronously. 3. Perform a detailed analysis of NHCS?s learning materials and programs; 4. Prepare and Present written report On-site (one day) 1. In addition topics to be covered during professional development seminar should include: blended learning techniques, support, opportunities accessibility and performance strategies. 5.2 Proposal Submission. Contractor shall submit a payment schedule as part of the cost proposal. 5.3 Travel: As part of the cost proposal, contractor shall provide any anticipated travel costs, if known to include: origination, destination, number of trips, number of persons, mode of transportation, lodging and meals must be accordance with Government Travel Regulations. 5.4 Reporting Requirements. The contractor shall inform the Government POC in writing of the POAM/Schedule. 5.4.1 The contractor shall bring problems or potential problems affecting performance to the attention of the Government POC as soon as possible. Verbal reports will be followed up with written reports when directed by the DAO. The provisions at 52.212-1, Instructions to Offerors Commercial Items is applicable to this procurement. Past Performance The offeror shall demonstrate relevant past performance or affirmatively state that is possesses no relevant past performance. Relevant past performance is performance under contacts or efforts (within the past three years) that is of the same or similar scope (e.g., Educational Consulting, Mixed method Qualitative and Quantitative research design for data collection and analysis etc), of the same or similar magnitude (e.g., performing services of a dollar value/FTE comparable to the dollar value/FTE of the offeror?s proposal), and of the same or similar complexity (e.g., The contractor will be required to provide an in depth review of instructional materials and delivery methods to that which is described in the RFQ. To demonstrate its past performance, the offeror shall identify up to three (3) of its most relevant contracts or efforts within the past three (3) years, and provide any other information the offeror considers relevant to the requirements of the solicitation. Offerors should provide a detailed explanation demonstrating the relevance of the contracts or efforts to the requirements of the solicitation. If subcontractor past performance is provided as part of the FIVE most relevant contracts, it will be given weight relative to the percentage of effort being provided by that particular subcontractor in the offer submitted. The references will be evaluated in the aggregate in order to allow offerors who may not have the entire scope, magnitude, and complexity of the requirement under one individual contract to still be considered acceptable if past performance with the full scope, magnitude, and complexity of the requirement can be demonstrated within the allotted number of references as described above. The offeror should complete a ?Past Performance Information Form? for each reference submitted. The form is an attachment to the RFQ. Only a single set of up to five references shall be submitted.
