- The Department of Health and Human Services, Program Support Center, Division of Acquisition Management, SAS, plans to award a simplified acquisition on a sole-source basis to CHD Research Associates, Inc., on behalf of the Agency for Health Research and Quality (AHRQ), Center for Quality Improvement and Patient Safety. The purpose is to analyze trends in national and state measures of hospitalization among home health and nursing home patients. Many home health and nursing home patients are hospitalized and in many cases, rehospitalized, after start of care, with resulting high costs. There is interest in examining these national and state hospitalization rates as measures of quality and disparity, especially the extent of state and agency/facility variation, and how these rates have changed over time. The overall hospitalizations rates will be further refined by potentially preventable indicators for use in the 2008 National Healthcare Quality and National Healthcare Disparities (NHQR/DR) Reports to Congress. Using existing (and updating files through latest available calendar year) home health and nursing home patient level files containing either Minimum Data Set (MDS) or Outcomes and Assessment Information Set (OASIS) assessment data merged with Medicare claims data, the following rates will be calculated: (a) Overall national and state rates hospitalization rates occurring within specified time periods after start of care for: i. Medicare Home health patients (calendar years 2000-2005); ii. Medicare Skilled nursing facility (SNF) patients (calendar years 1999-2005); iii. All nursing home residents with MDS assessment (regardless of being in Medicare SNF episode for 1999-2005). (b) National and state potentially preventable hospitalization rates (using the AHRQ Prevention Quality Indicators (PQIs) methodology and any additional ICD codes specified by the Government) to determine how many of the hospitalizations of home health and nursing home patients occurring within specified time periods after start of care are potentially preventable. These rates would be calculated for the 3 groups for the same time periods stated above. (c) Estimate actual PQI rates for home health and nursing home users and for important subgroups of users--for example, patients with a history of each PQI condition, a history of recent hospitalization, and users living in different geographic regions or being served by different types of agencies/facilities. The contractor may apply previous work (to be supplied by the Government) on risk adjustment for home health/nursing home-related hospitalization rates, in order to improve the validity of comparisons. (d) National and state potentially preventable hospitalization rates for the racial/ethnic groups used in the NHQR/DR (sample size permitting); (e) Overall agency- and facility-specific hospitalization and PQI hospitalizations rates for the specific time periods (for those providers with minimum sample size per year) for use in percentile rankings (e.g., top and bottom 10% or 25% for NHQR). CHD Research Associates, Inc. is uniquely capable of satisfying this requirement within the required time frame for the following reasons: 1. CHD Research Associates, Inc. is the key processor of this data for the Centers for Medicare & Medicaid Services (CMS) and is considered to be the expert on these files. They created these unique linked files (e.g., Datalink and DataPRO) and they have run analyses of the merged data sets previously and know the idiosyncrasies of this data; 2. They have continued to update these files, accounting for changes in Outcomes and Assessment Information Set (OASIS) and Minimum Data Set (MDS) so trends over time can be analyzed; 3. They have existing access to the CMS main frame to run these massive 100% datasets which cannot be run on PCs. The Agency for Health Research and Quality plans to use the nursing home and home health patient hospitalization measures for the 2008 National Healthcare Quality and National Healthcare Disparities Reports to Congress so these measures are needed by Spring 2008. This simplified acquisition is for services for which the Government intends to negotiate with only one source. Interested concerns may identify their interest in and capability to satisfy the requirement within 15 days after the date of publication of this notice. Information received will be considered solely for the purpose of determining whether to conduct a competitive acquisition. A determination by the Government not to conduct a competitive acquisition is solely within the discretion of the Government.
