Description
The South Texas Veterans Health Care System (STVHCS) is currently seeking sources of services. This is a sources sought synopsis only. This is not a solicitation announcement. No contract will be awarded from this notice. No reimbursement will be made for any cost associated with providing information in response to this notice. The proposed requirement mentioned herein is being considered for Service Disabled Veteran-Owned Small Businesses; Small Businesses; Small Disadvantaged Businesses, 8 (a); Women Owned Businesses; Veteran-Owned Small Businesses; Hub-zone certified; potential joint venture partners; or teaming partners. The government will use responses to this sources sought notice to make appropriate acquisition decisions.  The applicable North American Industry Classification System (NAICS) code is 621610 with a small business size standard of $12.5M. The resultant contract is anticipated to be Fixed Price Requirements with a twelve (12) month base period and the possibility of four twelve (12) month award terms. The contractor will be reimbursed for other direct costs incurred during the period of performance of the contract. Work will be ordered through the issuance of purchase orders. Contractor will provide to the STVHCS beneficiaries in all geographical locations served by the STVHCS storage, delivery, assembly, set-up, instruction, pick-up, sanitation, disposal, minor repair for reissue of durable and home medical equipment.  Deliveries will be made as such times and in such quantities as requested for DME and HME as specified in writing. Contractor must be JCAHO certified at time of award and maintain accreditation throughout the life of the contract. Contractor will provide service to approximately 81 counties in the South Texas region in and around surrounding areas of the following cities: San Antonio, Kerrville, Laredo, Harlingen, McAllen, Corpus Christi, Victoria, Eagle Pass, Del Rio.  Interested Service Disabled Veteran-Owned Small Businesses; Small Businesses; Small Disadvantaged Businesses, 8 (a); Women Owned Businesses; Veteran-Owned Small Businesses; Hub-zone certified; potential joint venture partners; or teaming partners should provide clear and convincing evidence that they are qualified to perform all of the services required. Standard company brochures are not desired. Responses should also include the following information: company name, address, point of contact, phone number and email address; Small Business or Small Disadvantaged Business status; 8 (a); Women Owned Businesses; Service Disabled Veteran-Owned Small Business; Veteran-Owned Small Businesses; Hub-zone certified status; potential joint venture partners or teaming partners. List of three references to which similar types of services have been previously provided to include contract number, dollar value, name and phone number of customer point of contact. The references must be relevant to the requirement and be within the last three year as of June 1, 2007.  The references must also include a description of the services rendered under that contract and demonstrate ability to provide services to service area. No feedback or evaluations will be provided regarding the information provided. This Sources Sought notice neither constitutes a Request for Quote, nor does it restrict the government to an ultimate acquisition approach.
