- The U.S. Forest Service is seeking information on sources for a national employee assistance program and a per employee cost (see below). This request for information is not a solicitation and the government is under no obligation to any party responing to this request for information. Statement of Work for the U.S. Service Employee Assistance Program INTRODUCTION ? To provide Employee Assistance Program (EAP) services to all U.S. Forest Service employees and their immediate family members. BACKGROUND ? The Forest Service EAP is designated to assist employees whose job performance or conduct is adversely affected as a result of personal issues such as alcoholism, drug dependency, mental or physical illness, financial or legal problems or domestic difficulties. SCOPE ? U.S. Forest Service seeks a contractor to provide EAP services to all of its approximately 30,000 employees located Nationally including those employees located in remote areas. TECHNICAL REQUIREMENTS ? Some of the requirements for this service would include, but no limited to, the following: -- Provide employees short-term counseling (between 6-8 visits) and/or referral for services such as alcoholism, drug dependency, mental or physical illness, financial or legal problems, or domestic difficulties. --To provide trainings to managers and supervisors on recognizing (not diagnosing) early signs of potential problems that could involve alcohol and drug abuse or other problems. -- Provide employee education awareness to inform employees of the counseling and referral services. -- All confidential laws and regulations governing the EAP program are to be followed. -- Provide standard EAP reporting requirements, such as quarterly reports. SUMMARY: The above information is to be used to obtain a quote for service. Additional information on EAP reporting requirements and billings, etc., will be provided at a later date, if applicable.
