Description
FedBizOpps FAC 2005-14 Q ? Medical Services This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, the test program in FAR Subpart 13.5, and FAR Part 15, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number 7TF-07-0009 Request for Proposal. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-14. This is a full and open procurement, after exclusion of sources, under NAICS Code 541511. NOTE: Section 508 does not apply to this requirement. Requirements: This requirement is for the acquisition of the following items: Medi-Span Database Information License, to include Average Wholesale Pricing and Patient Education information. Average Wholesale Pricing is an informational database which implements the pricing guidelines for pharmaceutical billing and reimbursement for the Agency nationwide. This annual licensing also provides for quality assurance measure, dosage checking, contraindications, allergy checking and patient education on the various prescribed medications. Patient Education is related information that provides a standardized patient medication information sheet that pharmacists must provide to patients. The items are to be delivered to the address below: Indian Health Service Office of Information Technology 5300 Homestead Road, NE Albuquerque, NM 87110 Attn: Mr. George Huggins Solicitation provision at FAR 52.21-1, Instructions to Offerors ? Commercial Items (JAN 2006) is hereby incorporated by reference. The proposal will be technically acceptable provided it meets the requirements of providing Medi-Span database information licenses and upgrades renewal that integrates with the RPMS application for Medi-Span Average Whole Sale Pricing, the Patient Education information and all the deliverables listed in paragraph 2.0.of this SOW. The Medi-Span database information and patient education information will be accessible for download across the internet to the Agency. Database and patient education information shall be able to provide quality assurance measure, dosage checking, contraindications, allergy checking on the various prescribed medications, and average wholesale pricing information. The Patient Education information shall provide a standardized patient medication information sheet that pharmacists must provide to patients in order to meet Omnibus Budget Reconciliation Act requirements. Technical Acceptability: Consist of the technical ability to provide the items as set forth in the solicitation requirement. Offeror?s are reminded to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications ? Commercial Items (June 2006) with its offer. Contractor must be registered in the Central Contractor Registration (CCR) www.ccr.gov and must have created a Marketing Partner Identification Number (MPIN) in the CCR. A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer?s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. (1) The provision at 52.212-1, Instructions to Offerors -- Commercial Items. This provision provides a single, streamlined set of instructions to be used when soliciting offers for commercial items and is incorporated in the solicitation by reference (see Block 27a, SF 1449). The contracting officer may tailor these instructions or provide additional instructions tailored to the specific acquisition in accordance with 12.302. (2) The provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items. This provision provides a single, consolidated list of representations and certifications for the acquisition of commercial items and is attached to the solicitation for offerors to complete. This provision may not be tailored except in accordance with Subpart 1.4. Use the provision with its Alternate I in solicitations issued by DoD, NASA, or the Coast Guard that are expected to exceed the threshold at 4.601(a). Use the provision with its Alternate II in solicitations for acquisitions for which small disadvantaged business procurement mechanisms are authorized on a regional basis. (3) The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items. This clause includes terms and conditions which are, to the maximum extent practicable, consistent with customary commercial practices and is incorporated in the solicitation and contract by reference (see Block 27, SF 1449). Use this clause with its Alternate I when a time-and-materials or labor-hour contract will be awarded. The contracting officer may tailor this clause in accordance with 12.302. (4) The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items. This clause incorporates by reference only those clauses required to implement provisions of law or executive orders applicable to the acquisition of commercial items. The contracting officer shall attach this clause to the solicitation and contract and, using the appropriate clause prescriptions, indicate which, if any, of the additional clauses cited in 52.212-5(b) or (c) are applicable to the specific acquisition. When cost information is obtained pursuant to Part 15 to establish the reasonableness of prices for commercial items, the contracting officer shall insert the clauses prescribed for this purpose in an addendum to the solicitation and contract. This clause may not be tailored. Use the clause with its Alternate I when the head of the agency has waived the examination of records by the Comptroller General in accordance with 25.1001. Type of Contract: A firm fixed price contract will be awarded. Delivery : June 1, 2007.
