Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION. Proposal s are requested and a written solicitation will not be issued. This solicitation is for award of a Blanket Purchase Agreements, not a contract. This combined synopsis/solicitation is being issued as a Request For Proposals. The provisions and clauses are those in effect though Federal Acquisition Circular FAC 2005-16 dated 22 Mar 07. This acquisition is solicited 100% small business set aside. The associated NAICS codes is: 622110. Estimated period of performance is 1 Jun 2007 to 31 May 2012. The Center for Health Care Contracting (CHCC) intents to award on a firm-fixed-price basis two commercial Blanket Purchase Agreements (BPAs) to provide personal services for locum tenens medical personnel in the Continental United Sta tes to include, Washington, D.C Proposals shall be submitted by 16 May 07 by 3:00 p.m. CST. It is anticipated that award will be made without discussions; however, the government reserves the right to hold discussions if necessary. Overtime: Applies to hours worked in excess of 8 hours per day. The overtime rate is also applicable for services rendered when required to report to the health care facility while on call. On Call: On-Call is defined, as a physician being required to carry a pager, be available for phone consultation and to report to the facility when called upon. While on-call, all physicians are required to remain within the community where they are work ing, and at no time should the provider be farther away than within 60 minutes from the facility. Should a facility have a requirement for a faster response time, the contractor shall accommodate with appropriate requirements. On-Call Night Physicians: Refers to a health care provider being on-call during weeknights off duty evening hours. This is a free to carry a pager, perform phone consultation and to be available to report to the facility when called upon. On-Call Day Physician: Refers to a health care provide being on-call for a full 24-hour period. This is generally performed on weekends and/or holidays during off duty hours. This is a free to carry a pager, perform phone consultat ion and to be available to report to the facility when called upon. The fee for an on-call day also includes up to the first two (2) hours of patient contact when required to report to the facility. On-Call Rates for Nurse Practitioners, Physician Assistants, Certified Registered Nurse Anesthetist (CRNAs) and Nurse Midwives: This covers carrying the pager only  phone consultation is not included, and to be available to report to the facility when cal led upon. All hours worked when called in to perform services are charged at the overtime rate. Proposals will be evaluated based on price and past and present performance, where price will be considered significantly more important than past performance. Offerors shall submit at least two references of contracts performed within the last three year s, which are the same and/or similar to this requirement. For each reference include: contract name, dollar value, contract office, agency name, contract number, period of performance, Contracting Officers name, telephone number and fax number. Past per formance does not have to be with a government agency. All responsive sources will be considered.
