AMENDMENT NO. 2 dated 4-23-07 - IB VA-250-07-IB-0001 Solicitation No. VA-250-07-IB-0001 dated March 23, 2007 is amended as outlined below: DELETION: On Drawing No. 06-117.02 New Architectural Plans 1st Floor, the construction of the wall adjacent to the "no work this area" is DELETED. This wall also runs adjacent to room R-37, R-38, R-39 and R-40. See ATTACHMENT "A". This wall will be an existing wall at time of construction but will require appropriate finishing on inside of wall within the specified rooms. CLARIFICATION - In accordance with the referenced clause 52.204-9 entitled "PERSONAL IDENTIFY VERIFICATION OF CONTRACTOR PERSONNEL (JAN 2006)" contained on Page 62 of the Specifications, the following verification procedures for contractor/subcontractors personnel are:: All employees must be finger printed prior to performing any duties at the worksite. Employees will report to the Building No. 1, Room 128 at the VA Medical Center, Chillicothe, Ohio. Each employee must present 2 forms of identification, i.e. driver's license, SSN card. NOTE: Fingerprinting is required regardless of any other security clearance that might be held by the employee and issued by another federal agency. All employees will be issued contractor badges which must be worn at all times while performing duties at the worksite. See ATTACHMENT "B" Policy Memorandum No. 07-04 dated February 7, 2007. CLARIFICATION - Any resulting contract will be subject to the Davis Bacon Act and Davis Bacon Wage Determination contained on Page 72 of the specifications. The Service Contract Act Wage Determination contain on Page 76 of the specifications is applicable to the Guarantee Period Services - See "Special Note" on Page 6 of the Specifications. CLARIFICATION - Page 64 - Referenced Clause 52.236-7 entitled "PERMITS AND RESPONSIBILITIES (NOV1991)" is hereby provided in full text as follows: "52.236-7 Permits and Responsibilities (Nov 1991) The Contractor shall, without additional expense to the Government, be responsible for obtaining any necessary licenses and permits, and for complying with any Federal, State, and municipal laws, codes, and regulations applicable to the performance of the work. The Contractor shall also be responsible for all damages to persons or property that occur as a result of the Contractor's fault or negligence. The Contractor shall also be responsible for all materials delivered and work performed until completion and acceptance of the entire work, except for any completed unit of work which may have been accepted under the contract. (End of clause)" CLARIFICATION: New Aluminum Store Front Vestibule New Aluminum store front vestibule is to be an 8x8x9 vestibule. Design entry to minimize accumulation of rain, snow and ice. Provide power door operators as shown. Design of entries should consider protection from exposure to wind. Bronze or grey colors are preferred at tinted glazing.  Provide clear glass at door glazing. Wind loading per applicable codes and per ASTM E330.  Deflection limit is to be 1/200 or limit of glass whichever is less. Air infiltration limit to 0.06 cfm/min/sq. ft. measured in accordance with AAMA 501.2 and ASTM E283. No water leakage - AAMA 501.2. Condensation Resistance Factor to be a minimum of 45 - AAMA 1503. Maximum U value for unit is to be 0.65, unless a life cycle analysis determines a lower value to be more cost effective. Warrantee for 5 years System to be fabricated and installed by a firm with a minimum of 3 years experience with the specified products.  Installers are to be approved by the manufacturer. Storefronts - extruded aluminum 6063-T5 frames and storefront framing, nominal wall thickness 0.125".  Thermally broken framing at storefront framing.  1" insulated storefront glazing.  Door glazing ¼" laminated tempered glass.  (Comply with applicable code requirements for safety glazing.)  Perimeter and blazing sealants. Doors - Door sections to be 1 ¾" tubular shapes of extruded aluminum 603-T5 alloy with 4" styles, 4" top rail and 12 -16" bottom rail.  Joinery shall be 3/8" diameter cadmium plated steel tie rods bolted through the stiles.  Where applicable a minimum of three rods will be installed.  Minimum wall thickness of the face is 0.100".  end wall thickness at the hinge and lock stiles to be 0.187" minimum.  Glass stops to be extruded angles with minimum wall thickness of 0.125" and to be removable fro the inside.  Doors to accept glass from ¼" laminated tempered glass.  Glazing in sidelights and transom to be 1" tempered insulated glass. Typical manufactures are Kawneer, Efco, Amarlite, Cross, Thompson, etc. Downspouts to be tied into existing drain at southwest corner of existing building above grade CLARIFICATION: spirit lighting can be found at http://www.lumens.com/lumens/search2.asp?search_freetext=tech+spirit&s_id=0&ad_id=ADW&key_id=spirit CLARIFICATION: Can lighting is Lithonia Model LF8 2/26-42TRT MVOLT F803A or equal. See Clause 52.211-6 entitled "Brand Name or Equal (AUG 1999)" contained on Page 8 of specifications. CLARIFICATIONS - GENERAL Window size for R-19 --- Nominal 2hx3w, HM frame, 36" AFF. Window size for R-13 ---12x12. Door frame details G-2 and G-3 are for the new opening between buildings, may have to be a split HM frame. Doors to receive new panic hardware per note 13, sheet 2 are aluminum and glass. Ambulance dock - build on existing walk, elevation same as existing building floor.  Railing and cover by same manufacturer as storefront. Handrail will be required on both sides of interior ramp between buildings.  Aluminum rails, ADA requirements and bolted to ramp or wall.  Do not extend rail beyond ramp next to steps. The plywood sub flooring is to level the area as much as possible.  It may require some of the second layer of existing wood flooring to be removed.  The sub flooring is necessary to receive the new tile flooring. There has been an asbestos inspection conducted and according to the owner of the building, there is no asbestos in the construction area. The new panel is to be fed from existing entrance metering enclosure and installed in the Elec. Closet, R-49. Skylights are type A, 2' higher than AT, enclosed with drywall (4 sides). R-29 is an eye exam room and needs to have dimming lights.  Therefore dimmer switch and dimmer ballasts Type B lights require 4 tubes. R-24 requires a recessed 2x4, 4 tube light fixture. New and existing Exit Lights are to be the same as the existing.  Use the same manufacturer and functions. VA will provide computer and telephone cable.  Contractor to install.  VA will terminate. All wiring, if not in conduit is to be plenum rated. Nurse call system is to be in ½" min. conduit Standard coaxial TV cable is to be installed (RG6) and come from the Comm. Room. The Mezz. is shown as R-14.  there will be a wall built along the edge of the floor as shown crossing the sink in the bathroom (R-16) and break room (R-15). Change all perimeter wall furring from 1" studs to 2 ½" studs. The three heat pumps are total electric.  Typical installation for a unit is #3/0 wire, 240/1/2, 60 amp --END--
