Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The contractor shall provide all management, labor, material, equipment, certifications and supplies necessary to provide physician assistance at the Department of Veterans Affairs, TOGAS, Augusta, Maine for 150 Maine Army National Guard Soliders in accordance with the Performance Work Statement (PWS). PERFORMANCE WORK STATEMENT (PWS)is as follows: MEDICAL SERVICES FOR PERIODIC PHYSICAL EXAMS (PHYSICIANS / PHYSICIANS ASSISTANTS 3-4 MAR 2007 FOR THE MAINE ARMY NATIONAL GUARD 1. Purpose 1-1. The purpose of this performance work statement is to describe medical services to be provided for the Maine Army National Guard (MEARNG) personnel during physical exams by physicians and/or physician assistants. The government my evaluate the quality of professional and administrative services provided, but retain no control over the medical professional aspects of services rendered. 1-2. To conduct medical services at Togus VA Hospital, 3-4 March for 150 soldiers, One VA Center, Augusta Maine 04330, from 0730  1630 each day. 2. Description of Services 2-1. The contractor shall provide all management, labor, material, equipment, certifications and supplies necessary, to provide medical services at Togus for the MEARNG. The contractor will provide services necessary to meet Department of the Army medical readiness standards to ensure mission capability of the MEARNG. 2-2. The contractor shall provide customized comprehensive physical examinations. These services will be completed using contractor provided equipment. 3. Exams 3-1. A physical evaluation shall asses the current state of physical health, risk for future medical disease as well as assessing general health factors that relate to the treatment of the patient. The following measures are required for each periodic phys ical evaluation. 3-1-1. Physical Examinations: The Medical Command will provide the contractor a current MEDPROS physical examination report on each soldier. The contractor will provide physicians or physician assistants to perform physical exams to ensure soldiers meet th e standards in accordance with Army Regulation 40-501, Standards of Medical Fitness, 27 June 2006. The contractor will utilize standard criteria for physical examinations as listed on DD Form 2807-1, October 2003, Report of Medical History, and DD Form 280 8, October 2003, Report of Medical Examination. Soldiers upon completion of exam will have a current five-year physical. The contractor will provide the completed physical to the Medical Command for MEDPROS data input and for the Soldiers medical record. The Medical Command will provide the physical examination forms (DD Forms 2807-1, 2808), physical examination rooms and tables, and laboratory exam rooms. 3-1-2. Immunizations: The Medical Command will perform all immunizations. 3-1-3. Laboratory (HIV): The Medical Command will perform all lab functions. 3-1-4. Laboratory (Physical Exams, age 38 and Older): The Medical Command will identify soldiers that require a Cardiovascular Screening (CVS) during their physical exam and provide a list to the contractor. Soldiers over 38 will receive a CVS. The Medical Command will draw a full lipid panel, H&H, glucose, and PSA (males only). The Medial Command will do a urinalysis on each identified soldier testing specific gravity, albumin, and glucose. Females will receive a pregnancy test. The Medical Command will co nduct an EKG. The Medical Command will provide all supplies, to include urine dipsticks, needles, tubes, pregnancy tests, packing supplies, and EKG Leeds. The contractors will ensure that all blood work and EKG reports are given to the Medical Command. Blood will be tested at Togus that day. 3-1-5. Laboratory (Physical Exams age 37 and younger): The Medical Command will draw blood on soldiers under 38 years of age. The blood test will check for H&H and cholesterol. The Medical Command will also conduct a urine test checking for specific gravit y, albumin, and glucose. Females will receive a pregnancy test. The Medical command will provide all supplies to include urine dipsticks, needles, tubes, pregnancy tests, and packing supplies. 3-1-6. Blood Pressure Assessment: The Medical Command will identify soldiers over 38 years of age that require blood pressure assessment. The Contractor will conduct a blood pressure assessment utilizing their own medical equipment to include stethoscope a nd sphygmomanometer. Screen for Hypertension 3-2. A MEARNG physician will review and co-sign all physical exams completed by the Contractor. 4. General Information 4-1. The government will periodically evaluate the Contractors performance in accordance with the Quality Assurance Surveillance Plan. 4-2. The Contractor shall act in accordance with DD Forms 254 (Department of Defense Contract Security Classification Specification). 4-3. The Contractor must maintain appropriate credentialing files on all employees that perform services in accordance with this contract. 5. Equipment and Waste Disposal 5-1. The contractor will leave all waste. The Togus staff will collect and dispose of all waste at the end of the day. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offeros: The best value proposal will be determined by evaluating the contractors offer based on Price and Past Performance and following technical factors in descending order of importance: 1)Contractors ability to meet the requirements the P WS. 2)Contractors experience with and knowlege of the Army Regulation 40-501. 3) Contractors ability to meet a three day response time. The technical evaluation factors and past performance, when combinded, are approximately equal in value to cost or price . Offerors shall ensure that the above factors are listed in their proposals. The provision at 52.212-4 Contract Terms and Conditions Commerical items, applies to this acquisition. Offerors shall include a complete copy of the provisions at FAR 52.212-3 Offeror Respresentations and Certifications Commerical items with its proposal. The provisions at FAR 52.212-5, Contract Terms and Conditions required to implement statutes or Executive Orders Commericial Item, are included and incorporated by reference.
