High St, Rm. 540, Columbus, OH 43215-2495 DESCRIPTION This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number AG-5540-S-0001. is issued as a Request for Quotes. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-15. The clauses and provisions referenced in this solicitation can be found in full text format at http://www.arnet.gov/far. This solicitation is 100% Set Aside for Small Business. The contract type will be firm fixed price. The associated NAICS code is 561720 and the small business size is $ 15 million. The following Department of Labor Wage Determination applies to this procurement: 2005-2421. The full text of the wage determination is included as an attachment. The United States Department of Agriculture, Farm Service Agency (FSA) announces a Request for Quote (RFQ) for Janitorial Services in the: Van Wert County FSA Office, 1189 Professional Dr., Van Wert, OH 45891. The leased property is 3,011 square feet with restroom space not included in that total square footage. Contiguous Counties, Auglaize and Hardin will require janitorial services, as well, solicited under separate cover as: AG-5540-S-0001 and AG-5540-S-0002, respectively. The Government will contemplate the award of multiple contracts to the same contractor, if it is in the best interests of the Government. When submitting a response, indicate the counties in which you have an interest. The Government anticipates the award of one or multiple Firm Fixed Price type of contracts resulting from this combined synopsis/solicitation. Responses should include the experience/qualifications of the contractor, personnel, equipment and services to perform the type of tasks described by this notice. High St. Rm. 540, Columbus, OH 43215-2495 STATEMENT OF WORK Statement of Work The contractor shall maintain the leased premises in a clean condition and shall provide supplies, labor, and equipment. Cleaning will be performed in 3,100 square feet of office space with included restrooms. Total carpeted space equates to 87%. The following schedule describes the level of services intended. Performance will be based on the County Executive Director??s (CED??s) evaluation of results, not on the frequency or method of performance. The CED will provide keys to the contractor. The service provider shall provide supplies (toilet paper, hand soap, trash bags, & hand towels), cleaning supplies and equipment. Storage closet is available if contractor needs to store supplies and equipment. Daytime cleaning is encouraged but interference with FSA business shall be avoided. The requested time to clean is from 6 A.M.-7:30A.M. Services Required A. Weekly (days to be determined by CED): ?X Empty trash ?X Vacuum all carpets and floor mats & sweep/mop tile floors. Remove carpet stains ?X Clean drinking fountain ?X Clean/sanitize all toilet fixtures & replenish toilet supplies ?X Dispose of all trash, re-cycling and garbage generated in or about the building on Tuesday??s. ?X Wash (inside and out) or steam clean cans used for collection of food remnants. ?X Dust horizontal surfaces that are readily available and visibly require dusting including Venetian blinds, light fixtures & heating/cooling vents. B. Every two Weeks: ?X Damp mop and spray buff hard and resilient floors in office space. C. Services Performed three times annually (March, July & November): ?X Wash windows inside & out that are within reach ?X Wash Venetian blinds, light fixtures & heating/cooling vents ?X Damp wipe toilet wastepaper receptacles, stall partitions, doors, window sills, and frames. Clean entrance and elevator carpets. ?X Dust wall surfaces within reach, vertical surfaces and undersurfaces. Clean metal and marble surfaces in lobbies. D. Services Performed annually: ?X Shampoo carpets in heavy traffic areas E. Services listed below require 7-day written advance notice to the government 1. Every six months: Wash all interior and exterior windows and other glass surfaces within reach. Dust vertical and horizontal blinds in the middle of the year long period between washings (see below). Spot clean carpet in main corridors and other heavy traffic areas. 2. Annually: Wash all vertical and horizontal blinds, Vacuum or dust all surfaces in the building within 70?? (178 cm) of the floor, including light fixtures. Vacuum all drapes in place. Clean carpets in corridors and lobbies. 3. As required: The vendor shall provide the government with written notice 7 days prior to performing unscheduled service and maintenance.
