- ADDITION, ALTERATION AND RENOVATION, CHINLE HEALTH CARE FACILITY, CHINLE, ARIZONA. The U.S. Department of Health and Human Services, Indian Health Service, Dallas Engineering Services Office intends to solicit offers for construction services necessary for the expansion and renovation of the Chinle Health Care Facility located in Chinle, Arizona. The Contractor shall provide all materials, equipment, labor and supervision necessary to perform the required construction work in strict accordance with the project specifications and drawings. The work consists of a base portion as well as six option items. Period of performance for the expansion portion of this project shall be twelve (12) months following receipt of notice to proceed. Period of performance for the renovation portion of this project shall be three (3) months following receipt of notice to proceed. A bid bond in an amount equal to twenty (20) percent of the offer is required. Contractors shall note that performance and payment bonds in an amount equal to one hundred (100) percent of the contract award amount will be required upon award. The North American Industry Classification System (NAICS) code applicable to this project shall be 236220. Applicable size standard shall be $31,000,000.00. In accordance with the Small Business Competitiveness Demonstration Program this procurement is hereby advertised on an unrestricted basis. Therefore, offers will be accepted from all responsive and responsible small and large business concerns. The estimated construction cost magnitude applicable to this procurement is more than $10,000,000.00. The Government intends to award a firm fixed price construction type contract to the responsive and responsible offeror whose proposal conforms to the requirements of the solicitation and will provide the best value to the Government. Proposals will be evaluated in accordance with the following evaluation factors. Contractors shall note that the evaluation factors provided herein are listed in order of relative importance. Evaluation Factors: (1) Past Performance: Construction experience on similar projects of similar size and scope. Elaborate on projects that have been completed in the last three (3) years. Provide references that may be contacted. OFFERORS SHALL SUBMIT THE OFFERORS QUALIFICATION STATEMENT FORM CONTAINED IN THE SOLICITATION DOCUMENTS; (2) Professional Qualifications and Experience of the Proposed Staff for This Project: Professional qualifications of project managers, superintendents, estimator, quality control, and construction inspectors including professional registration, education, training technical competence and skills as they relate specifically to medical projects. Individuals submitted for evaluation may not be switched and must be used in their capacity on the project; (3) Management Plan: Proposed project approach, construction method to accomplish contract requirements including teaming planning between contractor/sub-contractors, the designer of record and Government representatives. Cost management, critical path scheduling, value engineering and fast track experience must also be presented for consideration; (4) Quality Control/Quality Assurance/Safety Experience during Construction Phase: Description of specific safety and quality control initiatives and plans for this project. Provide information on your firm?s Quality Assurance/Quality Control procedures to ensure plans and specifications are complete and accurate, including coordination of drawings between disciplines. Show how firm ensures applicable codes and standards are met in the design. The construction contractor must provide a copy of the Contractors Experience Modifier Rate (EMR) for the last three years. (The EMR is a reflection of their safety performance as compared to the average contractor in their specific specialty and is calculated over a three year period. An EMR of 1.00 is average. This rating is used to determine the worker?s compensation insurance premium.) (This will also be a requirement for all sub-contractors when identified.); (5) Past Performance with Government and Private Industry Contracts of Similar Size, Scope, and Complexity: Past performance on contracts with Government entities including Native American organizations, tribal governments, federal agencies, i.e., Indian Health Service and Bureau of Indian Affairs and Private Industry. In accordance with FAR 15.305(a)(2)(ii), Offerors are required to identify past or current contracts (including Federal, State, and local Government and private enterprise) for efforts similar to the current Government requirements in this solicitation. Offerors are encouraged to demonstrate adequate experience, past performance, and technical ability to establish that their proposal will provide maximum benefit to the Government and meet the requirements of the specifications. The Government reserves the right to reject any or all offers at any time prior to award; to negotiate with any or all offerors; to award the contract to an offeror other than the offeror submitting the lowest price; and to award to the offeror submitting the proposal determined by the Government to be the most advantageous to the Government. Offerors are hereby advised that the Government may choose to award without discussions or any other contact concerning the proposals submitted. The solicitation package will be available on or about January 5, 2007.
