PLEASE READ THIS ENTIRE NOTICE CAREFULLY, AS IT CONSTITUTES THE ONLY NOTICE THAT WILL BE ISSUED. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation Subpart 12.6 as supplemented with additional information included in this notice. This notice constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The provisions of Subpart 13.5 Test Program for Certain Commercial Items are supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-13. The following provisions and clauses apply to this acquisition and are incorporated by reference: 1) 52.212-1, Instructions to Offerors-Commercial; and 2) 52.212-4, Contract Terms and Conditions-Commercial Items. Other clauses apply to this acquisition and full text can be found at http://acquisition.gov/far/index.html. Solicitation Number 1406-04-07-RQ-63513 applies; the solicitation is issued as a request for quote (RFQ). The US Department of the Interior, through its Franchise Fund activity GovWorks, will manage this procurement action on behalf of the US Department of the Air Force. The Government reserves the right to award without discussions. THIS IS A SET-ASIDE FOR SMALL BUSINESS. Business size will be verified using CCR. NAICS Code: 622110. Service Code Classification: Q. The RFQ is for an Occupational Health Specialist for Scott AFB Headquarters Air Mobility Command Surgeon's Office (HQ AMC/SG). The Contractor shall provide non-personal services for functional and technical support to the HQ AMC/SG in support of preventive and occupational health care for applicable beneficiaries. This includes expertise in the President's Safety, Health and Return to Employment (SHARE) initiative, OSHA's Voluntary Protection Program, and working knowledge in the prevention of work place injury. The Contractor shall assist in returning personnel to work through the review of occupational illness cases, recommend actions, and conduct medical reviews of controversial and complex cases. The Contractor shall develop, prepare, and conduct data collection on occupational health data for transition to useful information for HQ AMC/SG and other customers whereby medical prevention or intervention strategies can be implemented by customers to reduce workplace injury, occupational illness, and lost time. The Contractor shall provided health data expertise in the development of data tracking requirements for the collection and data analysis for use in identifying trends requiring intervention. The Contractor shall apply quantitative and statistical methods in analysis of data obtained.
