- The contractor shall provide acute and chronic hemodialysis, phoresis, and peritoneal dialysis services to the VAMC Louisville upon request of VA Staff nephrologists and approval by Chief, Medical Service and/or the Business Office Manager. In carrying out its responsibilities the contractor shall provide all appropriate training, equipment, including mobile reverse osmosis, phoresis and peritoneal dialysis machines. The contractor shall maintain such equipment. This is an unrestricted with small business preference (not a set aside)commercial items procurement. The following clauses and provisions plus any addendum apply: 52.212-1 Instructions to Offerors, 52.212-2 Evaluation, 52.212-3 Representations and Certifications and 52.212-4 Terms and Conditions and 52.212-5 Terms and Conditions to Implement Statutes and Executive Orders. The solicitation will be issued o/a 08/23/06 and may be downloaded at the following website: http://www.bos.oamm.va.gov/cgi-bin/WebObjects/BosPublic.woa All documents including amendments shall be downloaded by the offoror without further notice from the VA.
