- The GLAC (Great Lakes Acquisition Center) is soliciting quotations for PET or PET/CT Services for beneficiaries of the Department of Veterans Affairs, Jesse Brown VA Medical Center, Chicago, IL) for the period of December 1, 2006 through November 30, 2007, in accordance with all specifications, work statement, terms and conditions contained therein. All work will be performed at contractor?s facility. All qualified firms who believe they can satisfy the solicitation requirements are invited and encouraged to provide their proposal. Quotes shall be evaluated for fair and reasonable pricing, technical competency/quality of services and satisfactory past performance record. The Government anticipates award of a commercial item contract to the responsible quoter whose quote provides the best value to the Government. This is a Firm Fixed Price contract and this acquisition is a 100% Small Business Set-aside. The size standard for North American Industry Classification System (NAICS) code 621512 and $12.5M. Quoters are reminded to complete all sections of the solicitation, including the Offeror Representations and Certifications-Commercial Items. Requests for hard copy solicitation must be in writing to the Contracting Officer at the GLAC and accompanied by a check for $5.00 made out to the Department of Veterans Affairs. No telephone requests will be accepted. Any amendments issued to this solicitation will also be available at the URL address. Offerors are advised that they are responsible for obtaining amendments.
