- This FEDERAL BUSINESS OPPORTUNITY announcement is a notice that TRICARE Heartland Region (Region 5) that include all DOD Medical Treatment Facilities (MTFs) in the states of Ohio, Kentucky, and Illinois are in the process of standardizing Hospital Plastics as listed in the subject line above. Companies must obtain, or be in the process of obtaining, a Distribution and Pricing Agreements (DAPAs) with the Defense Supply Center Philadelphia for these items to be eligible to participate in the standardization process. Please respond by Email: phyllis.targett@wpafb.af.mil and provide your company name, address, point of contact, telephone, fax and E-mail address and answers to the below listed questions no later than 21 days from the posting date. For questions, please call (937) 257-9075. Technical Criteria 1. Does your company manufacture and/or distribute patient plastics (pitchers, drinking cups, emesis basins, bed pans, wash basins, urinals, sitz baths, etc) and not including specimen containers and collectors? If so, what types of plastics does your company manufacture? 2. Are your products available through the Prime Vendor, Owens & Minor? 3. What is your DAPA Number? 4. Under what brand(s) does your company manufacture or distribute? 5. Are your products latex free? 6. Do you use non-toxic,color fast dyes in your products? 7. How durable are your products? 8. Do your products have smooth edges? 9. Are your products sturdy and non-collapsible? 10. Is the use and function of the product easily identifiable? 11. Are numbers and characters on the product readable? 12. Do your products have an accessible area for easy application of the patient ID label? 13. Do your products come in different sizes? If so, what sizes? 14. Do your products come in different colors? If so, what colors? 15. Does your company provide customer service 24/7? 16. Is there any history of backorders and/or recalls for this product group? If yes, please answer: a. Dates and duration b. Cause c.
