- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Request for Proposal (RFP) number is DGOPR60116. This solicitation and incorporated clauses/provisions are those in effect through Federal Acquisition Circular 2005-12 effective 04 August 06 and Class Deviation 2005-o0001 and DFARS Change Notice 20060908. The solicitation incorporates by reference the following clauses: FAR 52.212-1, 52.212-3, 52.212-4, 52.212-5 Dev, 52.217-5, 52.217-8, 52.217-9, 52.232-17, 52.232-18, 52.232-33, 52.233-3, 52.247-34, and DFARS 252.204-7004 and 252.212-7001(dev). Clauses incorporated by full text: FAR 52.252-2 and 52.252-6 and DFARS 252.232-7010. NAICS Code: 541511. This purchase is a sole source requirement IAW FAR 6.302-1 to Dialogic Communications Corp. Supplies required are manufactured from only one source and no other type of supplies will satisfy agency requirements. This RFQ has the following line item(s): CLIN QTY SUPPLIES/SERVICES 0001 1 lot Contractor shall provide maintenance and technical support services for the Dialogic Communicator unit (Dialogic W2000 systems) in Bldg 12 (GNC) and the Indian Head, MD facility. Base Year: 1 October 2006 - 30 September 2007 0002 1 lot Contractor shall provide maintenance and technical support services for the Dialogic Communicator unit (Dialogic W2000 systems) in Bldg 12 (GNC) and the Indian Head, MD facility. Option Year One: 1 October 2007 - 30 September 2008 0003 1 lot Contractor shall provide maintenance and technical support services for the Dialogic Communicator unit (Dialogic W2000 systems) in Bldg 12 (GNC) and the Indian Head, MD facility. Option Year Two: 1 October 2008 - 30 September 2009 0004 1 lot Contractor shall provide maintenance and technical support services for the Dialogic Communicator unit (Dialogic W2000 systems) in Bldg 12 (GNC) and the Indian Head, MD facility. Option Year Three: 1 October 2009 - 30 September 2010 0005 1 lot Contractor shall provide maintenance and technical support services for the Dialogic Communicator unit (Dialogic W2000 systems) in Bldg 12 (GNC) and the Indian Head, MD facility. Option Year Four: 1 October 2010 - 30 September 2011 The maintenance and support services will ensure 24/7 operations of Dialogic W2000 systems that are used to notify DISA personnel in the event of an emergency, disaster or incident when operations must be relocated. Proposals must price each line item listed above (all or none). FOB Destination Ship to: Falls Church, VA 22041.
