- PERFORMANCE WORK STATEMENT FOR FARO SMX LASER TRACKING SYSTEM PREPARED BY: 76 MXW/OBWK August 2006 1. DESCRIPTION OF SERVICES. 1.1. Scope. The contractor shall provide all labor, tools, equipment, test equipment, personal protective equipment and clothing, ear plugs, safety glasses including pre-scription safety glasses, materials and parts, transportation and other incidentals necessary to travel to Tinker AFB, OK, to perform Preventive Maintenance (PM) (in-cluding software support) on one (1) FARO SMX Laser Tracking System and Reme-dial Maintenance (RM) (including software support) on one (1) FARO SMX Laser Tracking System. The contractor shall be responsible for insuring all services and parts provided under this contract are in accordance with the terms and conditions outlined herein and conform to the manufacturer?s equipment specifications and this Performance Work Statement (PWS). Equipment is listed in Appendix A. 1.2. Preventive Maintenance (PM). The objective of Preventive Maintenance (PM) is to protect the equipment capability and investment by removing causes of failure and making adjustments to compensate for normal wear before failures occur. 1.2.1. Pre-Preventive Maintenance (PM) Requirements. Using applicable manu- acturer?s specifications and industry standard practices, this Performance Work Statement (PWS), the contractor shall develop, concurrent with contractor?s quote, preventive maintenance instructions in accordance with AFMC 21-127, Depot Main-tenance Plant Management, paragraphs 2.3.1. through 2.3.1.3.3. and paragraphs 2.3.1.4. through 2.3.1.7.3. 1.2.1.1. The contractor shall provide, concurrent with the Contractor?s quote, Preventive Maintenance (PM) Instructions on AFMC Form 306, Preventive Mainte-nance (PM) Instructions. 1.2.2. Preventive Maintenance (PM) Schedule. Based upon the Preventive Mainte- nance instructions per the completed AFMC Form 306, Preventive Mainte?nance in-structions, the contractor shall develop and maintain a schedule to ensure all Preven-tive Maintenance (PM) tasks are scheduled and performed from onset of the contract period of performance. The schedule shall be given to The Quality Assurance Per-sonnel (QAP) within ten (10) working days after contract award and the contractor shall also notify the Quality Assurance Personnel (QAP) two weeks prior to each scheduled Preventive Maintenance visit. 1.2.3. Using the Preventive Maintenance (PM) Instructions submitted in paragraph 1.2.2., the contractor shall perform PMs (including software support) on one (1) FARO SMX Laser Tracking System and associated components to maintain maxi-mum, optimal operating specifications. 1.2.4. PM Documentation. The contractor shall document the completion of each Preventive Maintenance (PM) action in accordance with Technical Order 00-20-1, Chapter 7 and AFMCI 21-127, paragraphs 2.6.2 through 2.6.2.2. The AFTO Form 244 or AFTO Form 245, Industrial Support Equipment Record, can be found on the equipment, in the near proximity, or may be obtained from the Quality Assurance Personnel (QAP). The annotated form shall remain with the equipment. 1.3. Remedial Maintenance (RM). Remedial Maintenance is the evaluation, troubleshooting and diagnosis of equipment to isolate the source of a malfunction. Remedial Maintenance shall be expedited to minimize system downtime. 1.3.1. Malfunction Observation and Notification Process. 1.3.1.1. Upon discovery of equipment malfunction the Quality Assurance Personnel (QAP) shall request troubleshooting assistance (including software sup-port) by placing a telephone call to the designated contractor support point of contact. 1.3.1.2. The Quality Assurance Personnel (QAP) will provide a brief description of the malfunction and the name and telephone number of the equipment technical con-tact. 1.3.1.3. The contractor shall respond to the Quality Assurance Personnel (QAP) request for assistance within two (2) work hours after notification of a malfunction by contacting the equipment technical contact and troubleshooting via telephone com-munications. 1.3.1.4. If after 24 clock hours of troubleshooting via telephone communications, a resolution to the malfunction is not resolved, the contractor shall notify the Quality Assurance Personnel (QAP) that a site visit is required. The contractor shall travel to Tinker AFB, OK, arriving no later than 72 clock hours after discontinuance of trouble-shooting efforts. The contractor shall inspect the equipment/system as necessary to determine the cause of the malfunction. 1.3.1.5. The contractor shall prepare a written repair estimate using MXRIWRS Form 011, Repair Estimate Worksheet, or a contractor devised format providing, as a minimum the information cited on MXRIWRS Form 011. 1.3.1.5.1. The contractor shall submit the Repair Estimate Worksheet, MXRIWRS Form 011, to the Quality Assurance Personnel (QAP) on a best effort basis but no later than four (4) clock hours after completion of the inspection. In order to preclude the expense of a return visit, the contractor shall remain available up to two (2) work-ing hours, pending Quality Assurance Personnel (QAP), Administrative Contracting Officer (ACO) approval and repair taskings. 1.3.1.5.2. The Quality Assurance Personnel (QAP) will reviews the Repair Report and verify that sufficient funds are available to cover the cost of the repairs, permis-sion will then be given to repair the equipment in accordance with the terms and con-ditions outlined herein and conform to the equipment specifications published by the manufacturer. 1.3.1.5.3. When the contractor and the Quality Assurance Personnel (QAP) mutually agree that it is not feasible for the contractor to remain available pending repair task-ing, the contractor shall forward the written Repair Estimate Worksheet, MXRIWRS Form 011, to the Quality Assurance Personnel (QAP) within two (2) calendar days from completion of inspection. Upon completion of Quality Assurance Personnel (QAP) review the contractor shall schedule the repair work in accordance with the timeframes approved on the Repair Estimate Worksheet, accordance with the time-frames approved on the Repair Estimate Worksheet, MXRIWRS Form 011. 1.3.1.5.4. The contractor shall complete repairs in accordance with industry standard practices, manufacturer?s specification, this Performance Work Statement (PWS) and the approved Repair Estimate Worksheet, MXRIWRS Form 011. 1.3.1.5.5. Upon completion of the repair services the contractor shall demonstrate to the on-site Government Technical Point of Contact (POC) that all aspects of the sys-tem are repaired and are properly functioning in accordance with manufacturer?s specifications, industry standards and this Performance Work Statement (PWS). 1.3.1.6. Remedial Maintenance (RM) Documentation. The contractor shall document the completion of each repair action on AFTO Form 244 or AFTO Form 245, Industrial Support Equipment Record, in accordance with Technical Order (TO) 00-20-1, Chapter 7 and Chapter 7.11.6.10, 7.11.6.11. and 7.11.6.12. 1.3.2. Red X Documentation Symbol. A Red X indicates that the equipment is considered unsafe or unserviceable and will not be used until the unsatisfactory con-dition is corrected or the symbol cleared. No one shall authorize or direct equipment to be used until the Red X has been properly cleared in accordance with applicable technical data and Technical Order (TO) 00-20-1, paragraph 4.5. 1.3.2.1. Red X Special Certification Roster. The contractor shall provide to the Quality Assurance Personnel (QAP) by contract start date, a list of personnel who are authorized to certify that the equipment is safe for use in accordance with Technical Order (TO) 00-20-1, paragraph 1.3.2. 1.4. Parts and Material. Provide and use only new parts and material in perform- ance of this Performance Work Statement (PWS) unless authorized by the Contract-ing Officer CO) who will seek guidance from the Primary Quality Assurance Person-nel (QAP). 1.5. Service Report. Upon completion of each Preventive and Remedial Mainte- nance service call, the contractor shall provide a service report to the Quality Assur-ance Personnel (QAP). The service report shall contain the following minimum in-formation: Name of Company, Service Date, description of service performed, addi-tional service needed (if any), legible name and signature of technician performing the services, completion date of services, signature of on-site government technical point-of-contact (POC). 1.6. Emergency Contact. The contractor shall provide to the Quality Assurance Personnel (QAP) the name of the manager or designee who will be available by phone or pager to support contingencies and to respond to emergencies as required in paragraph 1.3.3, of this Performance Work Statement (PWS). 1.7. Security Requirement/Identification Badges. The contractor shall provide all delivery personnel requiring entry authorization to Tinker AFB OK, for performance of services associated with this requirement. The contractor and/or representative must have a valid driver?s licenses and car insurance verification. The following information is necessary to coordinate entry to the base. Company Name Full name of contractor and representative Cell phone # or pager # Estimated date and time of arrival Length of visit 1.7.1. The contractor and/or representative shall contact the Quality Assurance Per- sonnel (QAP) for directions to the base if unknown. After arriving at the designated gate?s parking area, the contractor shall telephone the Quality Assurance Personnel (QAP) for further instructions to be escorted on to the base. 1.7.2. Foreign National(s). Special rules apply for foreign national(s) visiting a US Gov- ernment facility. Please notify the Quality Assurance Personnel (QAP) before send-ing your technical representative out to perform services. 1.8. Availability of Equipment. Equipment shall be available for services Monday through Friday, 7:00 a.m. until 3:00 p.m., excluding Federal Holidays, unless other-wise directed by the Administrative Contracting Officer (ACO). 1.9. Environmental Protection & Safety. The contractor shall be knowledgeable of and comply with all applicable federal, state and local laws, regulations and require-ments regarding environmental protection and occupational safety and health. In the event the laws or regulations change during the terms of this contract, the contractor is required to comply as such laws come into effect. The contractor shall also comply with all safety practices posted in the assigned work area and the attached Appendix C, provided by the Safety Office. 1.10. Acknowledgment of ?Housekeeping/FOD/Tool Control Briefing for Contractor Employee?. Each contract employee shall acknowledge receipt of the ?Housekeep-ing/FOD/Tool Control Briefing for Contractor Employees? by reviewing, signing, dat-ing and returning MXRIWRS Form 032 to the QAP prior to starting work in 76 MXW Shops. The contractor lead shall notify the QAP when new employees are assigned. The contractor lead shall ensure each new employee receives and signs the ?House-keeping/FOD/Tool Control Briefing for Contractor Employees? before starting work in 76 MXW Shops. 1.11. Housekeeping. The contractor shall keep the work areas clean and neat in accordance with Industry Standards, Occupational Safety and Health Administration (OSHA), fire and safety standards and this Performance Work Statement (PWS). 1.12. Tool Control. While performing contract services in or around Maintenance Production shops, the contractor shall comply with AFMCI 21-107, Tool Control and Accountability Program and Tinker Instruction 21-112, Tool Management. Minimum requirements are: 1.12.1. All tools brought into the 76 MXW Wing areas shall be secured in a container or bag. 1.12.2. Each tool shall be identified and listed separately on a ?tool listing? kept in- side each container or bag. 1.12.3. Contractor personnel shall make the ?tool listing? available to government personnel when requested. 1.12.4. Contractor personnel shall ensure all tools are secured when not in use. 1.12.5. Contractor personnel shall ensure no tools are left unattended at any time. 1.12.6. Contractor personnel shall inventory the container or bag of tools at the beginning of the day. 1.12.7. Contractor personnel shall inventory the container or bag of tools at the end of each day. 1.12.8. Contractor personnel shall immediately notify the Quality Assurance Per- sonnel (QAP) of any tools that are missing. Tools not located within one hour of noti-fication will be reported by the QAP to the appropriate government personnel. 1.13. Foreign Object Damage (FOD) Prevention. While performing contract services in designated FOD Critical areas in or around Maintenance Production Shops, the contractor shall comply with AFMC 21-122, Foreign Object Damage (FOD) and Dropped Object Prevention (DOP) and Tinker AFB Supplement 1. 1.14. Continuation of Essential DoD Contractor Services During Crisis. In a state of emergency, if the government deems it necessary the contractor shall provide all services necessary to ensure mission accomplishment. On occasion, services may be required to support an activation or exercise of contingency plans outside the normal duty hours described above. The following requirements have been deter-mined to be essential in accordance with, DoDI 3020.27, E 2.1.1. and E 2.1.3: Per-formance Work Statement (PWS) paragraphs 1.3.1.3., 1.3.1.4. and 1.3.1.5.4. 1.15. Affirmative Procurement has determined not to be applicable to this Statement of Work (SOW). 1.16. Government Acceptance. Designated Quality Assurance Personnel will ac- complish inspection and acceptance of services performed based on requirements of the Performance Work Statement (PWS), the contract, the inspection of services clause and Quality Assurance Surveillance Plan. 1.17. The contractor shall comply with all applicable directives cited in this State- ment of Work (SOW). In the event the laws, regulations, Technical Orders (TO), Air Force Instructions, Air force Material Command Instructions change during the term of this contract, the contractor is required to comply as such changes come into effect at no additional cost to the government. 2. GOVERNMENT-FURNISHED PROPERTY AND SERVICES. 3.1. Security Police. The government will provide general on base security police service. Security Police telephone number is 911 for emergencies, 734-2000 for crime in progress and 734-3737 for non-emergency calls. 3.2. Fire and Emergency Services. The government will provide general on-base fire protection services. The contractor shall comply with fire prevention rules and regulations enforced by the Tinker AFB Fire Prevention Branch. Call 911 for emer-gencies. For routine fire prevention matters call 734-3981. For routine calls to dis-patch, call 734-7964. 3.3. Emergency Medical Service. The government will provide 24 hour on-base state certified paramedic level emergency response and patient transport services for con-tractor personnel who are injured or become seriously ill while on Tinker AFB, OK. The telephone number is 911 for emergencies. Note: Calling 911 from a cell phone will be received by off base emergency 911 system, causing a delay in response time by emergency personnel. When possible use a base telephone to receive on base help. 3.4. Workspace. The government will provide adequate workspace around the equipment area. 3.5. Collection. The government will allow non-hazardous only refuse to be emptied into the nearest authorized refuse dumpster for trash collection. Do not use the gov-ernment dumpsters for collection of construction debris generated by this contract. 3.6. Utilities. The government will furnish existing services located in the work area. The contractor shall use due care and diligence in efforts to conserve utilities to re-duce utility costs. 3.7. Forms and Publications. The contractor shall obtain the forms and publications expressly required to perform the work in this contract via the internet.
