Description

RESPONSE DATE IS SEPTEMBER 11, 2006 This amendment provides for the following: 1. Fire Emergency Vehicle Markings must be requested from Deanna Vest at 541-883-6737 (dvest@fs.fed.us) or Geraldine Wolfe at 541-883-6804. 2. Supply and Install has been extended to November 1, 2006 thru February 1, 2006. 3. Work can be accomplished at the vendors place of business, or at each Ranger District location listed. Please note on quote where work will be accomplished. 4. Successful quoter shall coordinate with Contracting Officer?s representative when work will be accomplished. 5. Response date is extended to September 11, 2006, 4:30 p.m. local time. This is a combined synopsis/solicitation to supply and install lights and sirens on 31 emergency fire vehicles. Request Fire Emergency Vehicle Markings sheet from Deanna Vest at 541-883-6737 (dvest@fs.fed.us) or Geraldine Wolfe at 541-883-6804) for information on how the vehicles are to be marked. There are eleven engines to be marked as Exhibit 1; 19 white pickups and SUV?s to be marked as Exhibit 2; and 1 green to be marked as Exhibit 3 as per the sheet referenced above. Installation shall be performed at the vendors place of business or at each District at the locations listed below. Quote shall designate where work will be accomplished. 4 at Chemult Ranger District, 110500 Highway 97 North, Chemult, OR 97731; 1 at Chiloquin Ranger District, 38500 Highway 97 South, Chiloquin, OR 97624; 3 at Klamath Ranger District, 2819 Dahlia St., Klamath Falls, OR 97601; 10 at Bly Ranger District, 61100 Highway 140, Bly, OR 97622; 4 at Lakeview Ranger District, 18049 Highway 395, Lakeview, OR 97630; 4 at Paisley Ranger District, 303 Highway 31, Paisley, OR 97636; 5 at Silver Lake Ranger District, 65600 Highway 31, Silver Lake, OR 97638. Supply and install shall be accomplished between November 1, 2006 thru February 1, 2007. This solicitation is a Request for Quotation (RFQ) using the policies prescribed in FAR Part 12, Acquisition of Commercial Items, in conjunction with the policies and procedures for solicitation, evaluation and award prescribed in FAR Part 13, Simplified Acquisition Procedures. This solicitation is a 100 percent Small Business Set-Aside. The North American Industry Code is 811121 and the small business size standard is not to exceed $6.5 million annual revenue. FAR CLAUSES AND PROVISIONS LISTED BELOW ARE AVAILABLE AT: www.arnet.gov/far/ Federal Acquisition Regulation (FAR) provision 52.212-1, Instruction to Offerors-Commercial Items applies to this requisition. Quotations will be evaluated using the evaluation procedures in FAR part 13. Quotations will be evaluated to determine the best value to the government. The following evaluation factors will be used: (1) Past Performance (CONTRACTOR TO SUBMIT REFERENCES INCLUDING CONTACT NAME AND PHONE NUMBER OF PREVIOUS CONTRACTS FOR SIMILAR INSTALLATIONS IN THE PAST 2 YEARS). (2) Pricing. FAR provision 52.212-3 Offeror Representations and Certifications-Commercial Items applies to this acquisition. OFFERORS SHALL SUBMIT A COMPLETED COPY WITH THEIR QUOTATION.
