DESCRIPTION OF AMENDMENT/MODIFICATION (Organized by UCF section heading, including solicitation/contract subject matter where feasible.) Solicitation VA-249-06-RP-0017 incorporates the following changes: Reference Paragraph 33 and 34 for Pharmacy: Paragraph 33 has no changes. Paragraph 34: for clarification "If emergent medications are administered in the clinic, the VA facility pharmacy will replace the used medications upon receipt of documentation of VA patient use". Routine meds are self explanatory as listed in paragraph 33. Delete the statement: The VA facility pharmacy will replace the used medications upon receipt of documentation of VA Patient use. Add the following: Emergency meds which should occur no more than 3 to 4 times a year may be provided by the contractor. Even emergency meds provided by the contractor will be no more than a 7-10 day supply. (this also is listed paragraph 34). Except as provided herein, all terms and conditions of this document referenced in Item 9A or 10A, as heretofore changed, remains unchanged and in full force and effect. 15A. Reimbursement will only apply to true emergency medications ordered for VA patients with documented visits to the contractor clinic on the day the emergency med was dispensed. A dispensing fee may apply and must be reasonable and in accordance with no more than the commercial pricing. The contractor must contact the COTR for authorization of a dispensing fee.
