Description
REFUSE COLLECTION PERFORMANCE BASED STATEMENT OF WORK Contractor shall provide a self-contained compactor with a minimum of 35 cubic yard container capacity. Compactor shall be operated with push button control, meeting all applicable OSHA Safety Requirements. Compactor shall operate with a system pressure of at least 2,200 PSI with a compaction force of 77,000 pounds minimum. Packer shall be hydraulic operated with minimum of 8 HP motor, Motor shall be 3 phase, 60 hertz, and 208 volts. Controls shall be 110 AC. Container shall have a holding chamber or sump to prevent liquid spillage. Should the contractor believe that any electrical work be necessary, he must notify the electrical shop 48 hours prior to starting. Contractor should also notify the Maintenance & Operations foremen at ext. 6907 along with the COTR. Workers should report to RM W629 to obtain badges and hot work permit if required, Pick ups shall be done between the hours of 9:00AM and 3:00PM Monday-Friday. Contractor shall meet all federal, state and city anti-pollution codes and requirements on disposal of the wastes that are being picked up. Contractor shall submit certification stating that the above regulations and requirements are being met. The dumpsite shall also be indicated. Paperwork must be submitted with the bid. Non submission of this information may make this bid non-responsive. Contractor shall pick up and empty the enclosed container located at rear of building 1. Contractor shall be responsible for any damages caused by technician to existing equipment and will be billed for repairs as a deduction on the invoice.. The same will apply for the Portsmouth site. All containers shall be covered during transport and delivery. No rolloffs can be delivered without covers. Contractor shall turn off the power switch to the compactor prior to removal and turn the switch on when packer units are completely hooked up. Contractor is not responsible for picking up any hospital medical wastes that require disposal by incineration. Contractor shall wash, clean and chemically disinfect the packer container inside and outside before returning to the site at least once per month or as needed. Compactor should be painted dark brown and its surface must be maintained monthly. If needs to be removed, then a temporary container must be provided with bird/vermin proof cover. Contractor is responsible for preventing any spillage from the ram hole while unit is being transported. The driver shall pick up any debris that falls between the container and the ram unit when picking the container up and put it back into the unit. We are recommending that the debris be broom swept.Contractor shall clean up any oil that leaks from the truck, using the appropriate measures to do this. If the VA is required to accomplish this oil removal task in order to prevent a potential danger to the oxygen supply truck, the contractor will be back charged a minimum of $100.00 cleaning fee.. The contractor will also be charged a $25.00 fee each time the VA is required to clean up debris that falls from the ram and the compactor when they are disconnected. A review of the site shall be made by appointment only with the Chief, Facility Service or his designee by calling (603)-624-4366 ext. 6156 during business hours. All fees, tariffs, and tolls shall be included in the bid price and be the responsibility of the contractor. The weekly delivery tickets shall be submitted with the monthly bill. The bill cannot be paid unless the delivery tickets are included. Unscheduled Collections-The contracting office may require contractor to make unscheduled collections, disposals and relocation of containers for special events or occasions. The contractor must respond with in 24 hours. These will be paid on separate delivery tickets. Inclement Weather Scheduling-Contractor is required to make pick ups during inclement weather. In case of severe weather, the contracting officer may grant exceptions. When these exceptions are granted, the contractor must make up the missed collection within 24 hours after the severe weather has terminated. ANY EXCEPTIONS TO THIS MUST BE GRANTED BY THE CONTRACTING OFFICER. Points of Collection- Contractor must position all containers so as not to interfere with parking areas, sidewalks, roadways, overhead utilities trees or any other potential obstructions. The Government may increase the size of container or frequency of pickups at any time. PERFORMANCE OBJECTIVES In order to establish an acceptable level for measuring performance the below guidelines are being established. Collection and disposal of the waste in according with established schedule-No more than 1 complaint per rolling six month period. Maintaining the equipment in working order and keeping all equipment in clean working order. Not more than 1 complaint per rolling six month period. Performing unscheduled pick ups- 0 deficiencies permitted. Keeping area in clean condition Not more than 1 complaint per rolling six month. period. QUALITY ASSURANCE The Government will appoint a representative who will periodically monitor the contractors performance. He will evaluate the performance based on on-site inspections and evaluation of complaints received.. The contracting officer may follow FAR 52.212.4 under Contract Terms and Conditions-Commercial Items should contractor fail to perform.
