Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this acquisition is F2Q4016131A200 and the solicitation is being issued as a request for quotation (RFQ). The solicitation document and its incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC 2005-11). The NAICS code for this acquisition is 811219 with a small-business size standard is $6,500,000.00. The 1st Contracting Squadron has a requirement to provide preventive maintenance agreement (PMA) service and emergency repair of six (6) processors for the main hospital and dental clinic at Langley AFB to include furnishing all labor, tools, parts, materials, facilities, and transportation required. Anticipated period of performance is October 1, 2006 through September 30, 2007 with one (1) additional one-year option and should be quoted as such with the option year priced separately. Pricing for all CLINs should be quoted as price per month for 12 months: CLIN 0001 PMA and emergency repair for Kodak Miniloader 2000 (Radiology); CLIN 0002 PMA and emergency repair for Kodak M6B (Radiology); CLIN 0003 PMA and emergency repair for Air Techniques AT2000XR (Dental); CLIN 0004 PMA and emergency repair for Air Techniques AT2000XR (Dental); CLIN 0005 PMA and emergency repair for Gendex GXP (Dental); CLIN 0006 PMA and emergency repair for Peripro III (Dental); CLIN 0007 Silver Recovery System and Wash Water Treatment System. Preventative maintenance includes providing all chemical developers, fixers, starter, and disposal of the same chemical when spent on routine monthly basis. Agreement includes routine monthly preventive maintenance, all service calls prior to and subsequent to regular preventive maintenance; chemistry replacement and disposal (fixer and developer) and wash water treatment (silver recovery). Services must result in acceptable radiological images meeting Food and Drug Administration (FDA), National Image Quality Standards, and applicable local/state/federal laws or regulations pertaining to hazardous waste disposal and recycling. The contractor shall have a valid Virginia state industrial waste permit. Disposing of any product or residue material after process into the sewer system is not authorized. The contractor shall perform all maintenance on site at 1 MDG, 73 Nealy Ave, Bldg 256, Langley AFB, VA. Work shall be performed between normal duty hours of 7:30 a.m. and 4:30 p.m., Monday through Friday excluding federal holidays. The Government will provide necessary replacement parts for contractor installation. The following FAR provisions and clauses are applicable to this procurement: 52.212-1, Instructions to Offerors—Commercial Items, is hereby incorporated by reference. Addendum to FAR 52.212-1, offerors must submit a copy of their valid Virginia State Industrial Waste permit along with a certification letter stating experience in servicing these types of equipment. Offerors must itemize and individually price according to CLIN numbers listed above. This is a best-value procurement. Award will be made to the offeror whose offer conforming to the solicitation is determined to be most advantageous to the Government, price and other factors considered. Technical and past performance, when combined, are equal to price. The Government intends to evaluate offers and award without discussion. In order to be considered for an award, an offeror must have completed the online electronic Representations and Certifications located at http://orca.bpn.gov/ in accordance with FAR 4.1201(a). The clause at 52.212-4, Contract Terms and Conditions – Commercial Items applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items applies to this acquisition. The following additional FAR clauses cited in 52.212-5 are applicable: 52.222-3, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.232-33, 52.222-41, and 52.222-42. Additionally, DFARS clauses 252.212-7001, 252.204-7004, 252.232-7003, 252.243-7001 and AFFARS clauses 5352.242-9000 and 5352.201-9101 are included. Additional contract requirements or terms and conditions determined by the contracting officer to be necessary for this acquisition and consistent with customary commercial practices. Refer to http://farsite.hill.af.mil/vffar1.htm to review applicable provisions and clauses. IN ACCORDANCE WITH FAR 52.232-18, FUNDS ARE NOT PRESENTLY AVAILABLE FOR THIS CONTRACT. THE GOVERNMENTS OBLIGATION UNDER THIS CONTRACT IS CONTINGENT UPON THE AVAILABILITY OF APPROPRIATED FUNDS FROM WHICH PAYMENT FOR CONTRACT PURPOSES CAN BE MADE. NO LEGAL LIABILITY ON THE PART OF THE GOVERNMENT FOR ANY PAYMENT MAY ARISE UNTIL FUNDS ARE MADE AVAILABLE TO THE CONTRACTING OFFICER FOR THIS CONTRACT AND UNTIL THE CONTRACTOR RECEIVES NOTICE OF SUCH AVAILABILITY, TO BE CONFIRMED IN WRITING BY THE CONTRACTING OFFICER.
