Description
The purpose of this modification is to 1) change the Level requirement of the MSW provider from Level II to Level I and 2) the number of years experience from one year to two years. The revised text is as follows: The Directorate of Contracting at F ort Drum, NY intends to make two (2) single awards to individuals or one (1) award to the responsible offeror for a personal service contract for New Parent Support Program Home Visitors in support of the Family Advocacy Program at Army Community Service a t Fort Drum, NY. The Home Visitor shall possess one of the following qualifications: (1) Must be a licensed social worker with an MSW from an accredited college or university and meet the standards of a Level I provide r as defined by DoDI 6400.1M, DoD, Q uality Assurance Standards. OR (2) Must be a registered nurse with a BA or BS in Community Health, Public Health, or Maternal and Child Health nursing and must possess and maintain a current, valid, unrestricted license IAW the laws of New York. Additiona lly, the Home Visitor shall have a minimum of two year experience working with adults and babies/young children in a social service or medical setting and one year experience providing services to clients in a military community or setting. This procureme nt is 100% set aside for small business and will be a Firm Fixed Price requirements contract consisting of the base year with four (4) option years. The applicable North American Industry Classification Code (NAICS) is 624190 with a size standard of $6.5 million. All offerors must be registered in the Central Contractor Registration (CCR). Any correspondence concerning this acquisition shall reference RFQ Number W911S2-06-T-0058. The anticipated issue/posting date of the solicitation will be on or about August 28, 2006. The RFQ and any subsequent amendments will only be available electronically by accessing either Federal Business Opportunities at http://vsearch2.fbo.gov/servlet/SearchServlet or the Fort Drum Contracting web page at http://www.drum.a rm y.mil/sites/directorates/doc-solicitations.asp.
