Description

Contractor to provide various Ophthalmology Services at the VA Black Hills Health Care System with facilities located in Hot Springs and Fort Mdeade, SD. Contractor shall be a board certified or board eligible Ophthalmologist with training in medical retina to include laser and cryopexy treatments. Contract will be for a one year base period commencing October 1, 2006 through September 30, 2007. Contract will include three aditional option years. Contractor shall perform on site Ophthalmology clinics and surgeries at times mutually agreed upon by both parties at both sites. Requests for the solicitation package must be in writing addressed to the Contracting Officer (LOG-CON-HS), VA Black Hills Health Care System, 500 North 5th Street, Hot Springs, SD 57747. Requests may also be faxed to the attention of the Contracting Officer at 605-745-2092. Offers are due on September 8, 2006 by close of business at 4:30pm. Applicable NAICS code is 621111. All responsible sources may submit a bid which will be considered. Subject to the availability of funds.
