Description
This amendment incorporates questions submitted by vendors for clarification. The responses to these questions are hereby incorporated into the Statement of Work as applicable. All other terms and conditions remain the same. Q. What are the two locations and what percent of the estimated 255 individuals will be done at each location? A. ? at MATES, ? at CSMS Q. What constitutes the 110 labs? Is it just a CBC, Chem 6, Lipid? Do you go by CRC guidelines? A. CBC, Chem 6, and Lipid profile Q. Can the 65 audiograms be done at one time, or one time at each location so we can utilize our mobile van? A. Audiograms are done in conjunction with exam, so no not at one time Q. Will the PFTs, EKGs and labs all be done in conjunction with the physicals? And will they all be done at one time? A. PFTs and EKGs with physicals, Labs all at once in the morning due to people having to fast Q. Is the physical a basis fit for duty physical? A. It is not a fit for duty physical Q. Regarding the 3 DOT physicals, will they also need a regulated UDS? Are they enrolled in a qualified Random Consortium? A. Q. Will the exams be done in large blocks or spaced out over the year? A. The period of performance will be completed at one time, preferably one week in September, the weeks of September 12, 19 or 26. Q. Do the audiometry portion of these exams have to be performed with DOHERS compatible software and is there any other software requirements for any other component of the exam. A. DOERS-HC software is required for audiometry. No other software requirements for the other parts of the exam. Q. Who was the vendor for previous contract? A. The previous contract was awarded to Professional Health Services. Q. What was the total award amount and the dollar amount for each CLIN for the previous contract? A. Total award amount for the previous contract was $48,870.00 under a best value selection process. Release of individual CLIN pricing is prohibited by federal regulation without a FOIA request. Q. How many hours of consulting did the COHN actually provided on the last contract? A. Consultation required a site visit for coordination prior to the performance period. As the consultation was not paid for or invoiced separately under a line item, the government has no way of knowing the number of hours. Q. When preparing pricing for the bid, which pricing sheets should be used, page 7 and 8 or page 9 through 13? On page 9, item no 001 there is a line item for mobile medical testing with no unit, just a line for Net Amount. Then on page 10 item no 001AA is the physical exams. It item no 001 for a flat fee? A. Pricing for individual tests are to be listed on the pertinent line item. CLIN 0001 is a roll up of CLINs 0001AA-AJ. Q. As the physical exam requires a hearing test, is the quantity of 65 audiometric tests in CLIN 0001AC accurate? A. Yes, the government estimates 65 audiometric tests for soldiers not requiring a full physical.
