Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice for the South Texas Veterans Health Care System (STVHCS). This announcement constitutes the only solicitation; Request for Quote (RFQ) 671-34Q-06, a written solicitation will not be issued. Solicitation documents and incorporated provisions and clauses are those in effect through federal acquisition circular 2005-11. NAICS 621320 with business size standard $6.5M. This solicitation is 100% small business set-aside. STVHCS has a requirement for eye exam services for the period of date of award through December 31, 2006 for beneficiaries in locations within the South Texas Veterans Health Care System, to include but not limited to San Antonio, Harlingen, Corpus Christi, Victoria, Laredo, McAllen, and Kerrville. The Government intends and reserves the right to make multiple awards. Offers should be submitted on letterhead paper and address the following criteria: Cost for each eye exam; all contractors and all sub-contractors must have an active and unencombered Texas Optometric License or Texas Medical License as issued by their respective Licensure Boards, must not have an active malpractive litigation filed against them nor have an active Texas Optometric Board or Texas Medical Board investifation theat is ongoing, must not have any sanctions or pending sanctions against their license. All contractors must have facilities equipped with appropriate equipment to perform a complete dilated eye examination to include phoropter, slit lamp, tonometry, indirect opthalmoscopy, and any other necessary equipment. Offerors must provide copies of licenses and certifications along with the offer. Contractor will provide quality assurance reports upon request, employ highly experienced personnel and have monitored and controlled clean room conditions. Contractor will provide weekly invoices with complete itemized list of all procedures performed during the week to include: (1) name of veteran patient treated on the original letter/voucher that patient presents to your office, please be sure that the last four ( 4 ) digits of his/her SSN are documented (2) the original eye examination medical record ; and, (3) when the patient should be seen again is to be noted in the medical record. Oral Quotes will not be accepted, faxed quotes must be followed up with a verbal confirmation of receipt.
