Description
THIS IS A SOURCES SOUGHT ANNOUNCEMENT, NOT A REQUEST FOR PROPOSAL. Please FAX a note of interest to 801-584-2506. Do not call. You may email questions to Richard.garrard@va.gov. The George E. Wahlen Veterans Affairs Medical Center in Salt Lake City, Utah is seeking interested QUALIFIED AND EXPERIENCED SMALL BUSINESS sources currently licensed and providing Hazardous Material/Hazardous Waste Disposal services in the Salt Lake City, Utah area. If your company is a small business which can meet the requirements below, please fax your information with your small business status clearly indicated, your DUNS number and point of contact information. If no interest is received, the solicitation will be issued on an unrestricted basis (open to large business as well). Contract Requirements: a. Contractor able to respond to VA emergency within 24 hours. Be physically present and advise VA what to do if we have possible chemical explosive or other type of emergency. Contractor would help prepare a plan to fix problem in cooperation with VA. b. Contractor office located in State of Utah as per response requirements. Some HW disposal may be so urgent we need to make arrangements to dispose of it right away. This may or may not occur with emergency noted above. We may have some HM or HW storage deteriorate to the point of needing immediate disposal. c. Contractor able to provide documentation he is licensed to transport hazardous waste in Utah, including compressed gas cylinders. d. Contract able to provide appropriate documentation that he can transport infectious waste commingled with HW in Utah. e. Contractor able to provide appropriate documentation of ownership of permitted Resource Conservation and Recovery Act Treatment, Storage and Disposal Facility (TSDF) within the United States. f. Contractor able to provide documentation that he has appropriate permits, licenses, etc to dispose of hazardous waste in location of TSDFs appropriate to chemotherapy waste collection and disposal; U listed hazardous waste that is also antineoplastic waste, to include containers (IV bags, drug bottles, sharps, etc) which should be assumed to contain more than 3% chemo residue. g. Contractor able to provide documentation that his staff transporting, treating or disposing of HW and infectious waste has received all regulatory required training. h. Contractor able to provide documentation he has appropriate equipment and tools to provide safe environment for their staff working with HW. Equipment includes appropriate transportation vehicles, placards, labels, packing materials, etc. 1. Contractor able to provide Department of Transportation (DoT) specified containers for HW transported from VA property and documentation of appropriate material handling equipment. i. Contractor able to provide HW consultation regarding appropriate storage, packing, labeling, etc. j. Contractor able to demonstrate track record of consistent customer satisfaction, EPA, OSHA, state and local compliance, reliability, responsiveness through references and other documentation. k. Contractor able to provide Certified Hazardous Material Management Onsite Training.
