Description
Under the authority of Public Law 104-262 and 38 U.S.C. 8153, the VA Medical Center located at 7305 N. Military Trail West Palm Beach, Florida 33410-6400 has a requirement to award a single or multiple award contract for the delivery of Primary Care and Mental Health Services to eligible beneficiaries of VA Medical Center for the period on or about December 1, 2006 through September 30, 2007, pending availability of FY ?07 funding, with four one year options to renew. The total duration of this contract shall not exceed fifty eight (58) months. This solicitation is issued as a commercial item in accordance with FAR Part 12 and shall be evaluated in accordance with FAR Part 15. Request for Proposal (RFP) 548-06-485 is advertised with NAICS: 621111 and a size standard of $9 million. The Government intends to award an indefinite delivery, indefinite quantity, firm fixed price contract in accordance with all terms, conditions, provisions, specifications and schedules of the solicitation. Each offer should contain the offeror's best terms from a cost/price and technical standpoint, as contract award may be based on initial offers received without discussions. Primary care & mental health healthcare services for veterans in their local communities. Communities served under this contract are the Southern Palm Beach County, St. Lucie County, Martin County, Indian River County, and Okeechobee County. The clinics shall be located in Delray Beach and Boca Raton; Ft. Pierce; Stuart; Vero Beach; and the town of Okeechobee respectively. The contractor shall provide for the continuous delivery and management of primary care & mental health services for all assigned VA patients. Referrals for specialty care, extensive diagnostic work-ups and non-emergency hospitalization shall be made to the VA Medical Center (VAMC). There are an estimated one-hundred twenty-seven thousand (127,000) veterans residing in the areas covered by this contract. Age demographics are: Age<45 Age45-64 Age65+ S. The most common outpatient diagnoses treated include diabetes mellitus, hypertension, atrial fibrillation and atherosclerotic heart disease. The most common DRGs for patients moving to the acute care setting are congestive heart failure, chronic obstructive pulmonary disease, artherosclerosis, angina, and cardiac arrhythmia and conduction disorders. VA estimates that each patient will make three visits per year to the contract primary care facility. This amount is an estimate and is to be used for information purposes only. The contractor shall provide health care providers, medical facilities, medical equipment and supplies, specified pharmaceuticals, and all administrative functions to fulfill the level of care and range of services as may be required by the VA Medical Center and in accordance with Joint Commission on Accreditation of Healthcare Organizations (JCAHO), or other regulatory agencies. The solicitation will issue on or about July 14, 2006. A Pre-Bid Conference will be held on July 27, 2006 at 8:30 am in Room 3B-260 at the VAMC West Palm Beach FL. Any and all questions shall be submitted in writing no later than 12:00 noon, on July 25, 2006. All attendees must sign the attached consent form, Consent for Use of Picture and or Voice, VA 10-3203, and return via FAX to (561) 422-6507 no later than July 26, 2006 at 4:00 pm., since the Pre-Bid Meeting will be videotaped. The closing date for submission of proposals will be on or about August 11, 2006 at 2:00 PM local time. No telephone or fax requests for solicitation will be accepted.
