- THE PURPOSE OF THIS AMENDMENT IS TO CHANGE THE SYNOPSIS FROM 100% SMALL BUSINESS SET-ASIDE TO UNRESTRICTED AND CHANGE THE SOLICITATION ISSUE DATE FROM ON OR ABOUT 5 JULY 2006 TO ON OR ABOUT 11 JULY 2006. This is a synopsis for a commercial acquisition using simplified acquisition procedures (Reference FAR Parts 12 and 13). THIS IS A SYNOPSIS AS REQUIRED BY FAR PART 5.2 ? THIS IS NOT A REQUEST FOR QUOTE/PROPOSAL. VENDORS SHOULD NOT SUBMIT FORMAL QUOTES/PROPOSALS UNTIL THE RFQ/RFP IS POSTED TO THIS WEBSITE. 45 CONS/LGCB has a requirement for laboratory transportation services at Patrick AFB (PAFB), Florida. The contractor will be required to pick up and deliver laboratory specimens from PAFB, FL hospital as required and deliver to Keesler AFB hospital, laboratory, Keesler AFB, Mississippi and other specified locations, as required. In addition, the contractor shall pick up and deliver medical supplies to Medical Logistics, PAFB, FL. The contractor is not responsible for the return shipment from Keesler AFB, MS or the other various locations to PAFB, FL. Items to be delivered include surgical and cytology specimens. Approximately two specimens per week shall be required for delivery to Keesler AFB, MS. Daily cytology shipment to AFIP Washington, DC, pathology specimens to Epi (Brooks AFB, TX) and Wilford Hall (Lackland AFB, TX) two times a week. The contractor shall respond within one hour after a call is placed normally between the hours of 7:30 a.m. ? 4:30 p.m., Monday through Friday; however, a requirement may exist for weekend/holiday delivery. Occasionally, a requirement may exist to deliver the specimens to another location other than Keesler AFB, MS; however, these occasions are very rare. This service will be conducted at Patrick AFB, FL. The period of performance is 01 October 2006 ? 30 September 2007. Four one-year options may be awarded. The government intends to make a single award to the lowest priced, technically acceptable offeror. This proposed procurement is a 100% small business set-aside. Solicitation No. FA2521-06-T-0183 is issued as a Request for Proposal. It is advertised with a Standard Industrial Code (SIC) of 7389, business size of $6.5M and a North American Industry Classification System (NAICS) code of 491110. GSA Schedule holders for this item/service are encouraged to provide us with their GSA Schedule number. All responsible sources may submit a proposal, which shall be considered by the agency. The solicitation will be issued electronically on or about 5 July 2006 at www.fbo.gov. Proposals may be submitted via hard copy, fax (321-494-5136), or e-mail.
