Description

Travis AFB intends to award a sole source purchase order for 40 packs of Custom Added Vue Cards, P/N 0030-4835 to be used to the VISX Excimer Laser, serial no. 5147. VISX USA, Inc. is the manufacturer of the equipment currently in use at David Grant Laser Refractive Center, and their products are the only compatible products with the VISX brand. Delivery is to be made to DGMC, 101 Bodin Circle, Bldg. 777, Travis AFB, CA, 94535. Required delivery of the product is 30 days. All responsible sources may submit a quotation, which shall be considered by the agency. NAICS code 339112 applies. SIC code 3841 applies. Contractors must be actively registered with the Central Contractor Registration (CCR) at http://www.ccr.gov and at the Online Representation and Certification Application (ORCA) site at http://orca.bpn.gov.
