- This is published as a sole source announcement under the authority of the test program for commercial items (Section 4202 of the Clinger-Cohen Act of 1996). The proposed contract action is for commercial services for which the Government intends to solicit and negotiate with only one source pursuant to the authority FAR 13.501(a)(1)(ii). See Note 22. The NAICS code for this action is 325412. The Fleet and Industrial Supply Center (FISC) San Diego, CA intends to procure the following supplies from Baxter Healthcare Corporation. Clin 0001: 450 ML CPD/ADSOL Triple BPU Baxter Healthcare Part Number: 4R3431NM (30 per case) Qty: 200 cases Clin 0002 SAMPLINK Access Device Baxter Healthcare Part Number: 4R4215 (60 per case) Qty: 200 cases Salient Characteristics: SAMPLINK Access device for use with Blood-Pack Units with Lab site sampling site Clin 0003: 450 ML CPD/ADSOL Double WB Pack with filter Baxter Healthcare Part Number: 4R3303NM (20 per case) Qty: 420 cases All supplies shall be delivered in accordance with the following schedule: Clin 0001 and 0002/ 20 cases per delivery Clin 0003/ 35 cases per delivery Dates: 02 Oct 06 02 Apr 07 01 Nov 06 01 May 07 01 Dec 06 01 Jun 07 01 Jan 07 02 Jul 07 01 Feb 07 01 Aug 07 01 Mar 07 04 Sep 07 Aforementioned supplies is for the Blood Donor Department in the Naval Medical Center, San Diego, CA. Interested vendors may identify their interest and capability to respond to the requirement or submit proposals. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-09, effective 19 May 2006 & Class Deviation 2005-o0001 and DFARS DCN 20060519 edition. It is the contractor?s responsibility to be familiar with the applicable clauses and provisions. The provision at FAR 52.212-3, Offeror Representations And Certifications ? Commercial Items, (January 2004) Alternate I (April 2002) applies. This provision is a fill-in and Offerors must complete all applicable portions and submit the completed copy with its quote. To obtain a copy, please contact Ms. Evelyn Atienza. NOTE: Vendor is required to use the web-based Online Representation and Certifications Application (ORCA) for this solicitation. If you are currently registered in the Central Contractor Registration (CCR), access your account in CCR, and enter a Marketing Partner ID Number (MPIN). Then login to ORCA to add your ORCA record. The ORCA website is http://orca.bpn.gov/login.aspx. Your MPIN takes 24 to 48 hours, after your registration in CCR, to be activated in ORCA, so you are encouraged to complete this entire process in sufficient time so that your proposal can be submitted on time. The Offeror shall submit a quote that include firm-fixed pricing, FOB point, delivery time, payment/discount terms, all applicable technical documentation and information required by the solicitation. The quote shall also provide a point of contact, name and phone number. This notice of intent is not a request for competitive proposals. However, the Government will consider all statements of interest and capability received no later than 8 Jun 2006. A determination by the Government not to compete the proposed contract based on responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. All statements of interest/capability documentation must be submitted via email attachment or fax to the attention of Ms. Evelyn Atienza at 619-532-4957. Mailed documents will not be accepted.
